[Gynecomastia]Changes 7 months after gynecomastia surgery

Hello, this is Dr. Kukhan Lee,
who is in charge of gynecomastia surgery
at Lesarts Plastic Surgery.

One of the most common questions
from patients considering gynecomastia surgery is,
“How much change can I actually expect after surgery?”

Even after reviewing before-and-after photos
or detailed explanations,
many patients hesitate
because they are unsure
whether the surgical results
will truly apply
to their own body.

This concern is especially common
for those who have experienced
long-term stress from chest protrusion,
as anxiety about the postoperative appearance
tends to be even greater.

In this article,
we will explain the effects of gynecomastia surgery
and the key points of change
through a real patient case
7 months after surgery.

This case involves a 27-year-old male patient
who experienced persistent chest enlargement
and related stress since adolescence.

Despite consistent exercise and weight control,
chest protrusion, especially around
the areola area and lower chest,
did not improve easily,
leading him to suspect gynecomastia
and seek consultation.

Through physical examination
and ultrasound evaluation,
the patient was diagnosed with
true gynecomastia, Simon grade IIa.

Based on this diagnosis,
a subcutaneous mastectomy
was planned as the appropriate treatment.

Before surgery, the upper chest shows
a well-developed pectoralis major,
creating a relatively masculine chest contour.

In contrast, the areola area and
lower chest appear noticeably protruded,
revealing an overall silhouette imbalance.

From the side view,
the most prominent point is not
the upper chest,
but the periareolar area instead.

As a result, even when wearing clothes,
this area makes contact first,
making the protrusion more noticeable.

The patient also reported that
chest protrusion became most noticeable
whenever wearing thin clothing,
causing significant daily discomfort.

Because gynecomastia surgery
removes only the glandular tissue
without affecting the chest muscles,
there is no need to worry about
muscle strength loss
or muscle volume reduction.

At 7 months after gynecomastia surgery,
most biobond materials have been absorbed,
and the tissue becomes fully stabilized.

At this stage, the chest shows
a clearly different appearance
compared to before surgery,
with a more natural, balanced chest contour
now clearly visible.

Through gynecomastia surgery,
chest volume reduction was achieved,
and areolar protrusion was also improved.

The chest that once appeared
rounded and protruded before surgery
has become overall flatter and balanced.

From both side and oblique views,
the point that first touches clothing
has shifted from the areolar area
to the upper chest,
clearly confirming a natural contour change.

By removing glandular tissue around the areola
while preserving upper chest volume,
the highest point of the chest
naturally shifts upward.

As a result, even in thin clothing,
the chest no longer appears protruded as before,
creating a cleaner, more masculine impression.

As the lower chest height
from the inframammary fold
to the areola decreases,
the vertical length of the chest
becomes noticeably shorter.

As a result, the chest appears
firmer, more compact,
and distinctly masculine overall.

Even with gynecomastia surgery alone,
a sufficiently natural improvement
can be expected in most cases.

After full postoperative recovery,
continued exercise to further develop
the pectoralis major muscle
can help create a more balanced chest line.

Gynecomastia surgery is not
simply about reducing volume,
but a process of reshaping
the chest silhouette and impression.
