Body Fat Percentage: 29.4%

The patient was married
and had a history of childbirth.

She worked in an office setting

and consistently attended
Pilates classes

despite her busy schedule.

However,

even with her dedication
to exercise,

her thighs remained noticeably larger
than her upper body.

In particular,

the inner thighs
and saddlebags

were highly developed,

giving her lower body
an appearance similar to that

of an athlete.

As seen in the preoperative photos,

the inner thighs
were in constant contact,

creating a crowded appearance

with no visible thigh gap.

The prominent saddlebags
also made the legs appear

shorter and thicker
than desired.

In addition,

the front of the thighs
projected outward significantly,

making the thighs appear
especially bulky

when viewed from the side.

Ultrasound measurements showed
substantial subcutaneous fat deposits:

Outer thighs: 34 / 32 mm

Inner thighs: 19 / 19 mm

Front thighs: 17 / 18 mm

Back thighs: 13 / 14 mm

Inner knees: 12 / 10 mm

Based on these findings,

we expected the patient
to achieve a favorable outcome

from thigh liposuction.

The patient’s primary concerns

were the inner thighs
and outer thighs,

and her goal was
to achieve a smooth,

naturally contoured leg line.

Key Design Principles for Thigh Liposuction

Our approach was to remove

as much excess fat as possible
throughout the thighs,

while intentionally preserving fat

in specific areas
that contribute

to a smooth
and balanced contour.

For example,

in the inner thighs,

the region extending
from above the knee

to the mid-thigh,

as well as the transition area
between the thigh and buttocks,

requires careful preservation.

Removing all of the fat
from these regions

can create an unnatural break
or indentation in the contour.

Particular attention was given

to the area beneath
the buttocks.

Not only would excessive fat removal
compromise the appearance,

but this fat also provides support
for the buttocks.

If removed completely,

the buttocks may lose support
and begin to sag.

For this reason,

fat in this area
was intentionally preserved.

However,

because the patient had
a relatively large amount of fat

and reduced skin elasticity
along the inner thighs,

we explained in advance

that some degree
of skin laxity

could develop following surgery.

The upper portions
of the outer thighs

and front thighs

were also treated conservatively.

If too much fat is removed,

the underlying muscle contours
can become visible,

creating hollowed areas.

Therefore,

a small amount of fat
was intentionally left behind

to maintain smooth
and natural transitions.

With the removal of fat
from the inner thighs,

the patient developed

a visible thigh gap
that was completely absent

before surgery.

As a result,

the legs appeared
much lighter,

more refined,

and visually longer.

However,

as discussed before surgery,

a small degree of skin laxity

was observed along
the upper inner thigh

and groin area.

On the outer thighs,

the reduction of
the saddlebags

created a smooth,
continuous curve

from the knees
to the pelvis,

contributing to a slimmer
and longer-looking leg contour.

The front thighs
were also refined

through targeted fat removal.

The previously rounded
and protruding contour

was transformed into
a softer,

more gradual curve,

making the legs appear
