Thigh Liposuction: Completing the Final 2% of Body Contouring
Hello, this is Dr. Yongseung Seo
from Lesarts Plastic Surgery.
Liposuction is more than
a simple body contouring procedure.
Even after years of dedicated exercise
and dietary control,
it can precisely remove
the “last remaining fat”
that stubbornly persists,
revealing the beautiful skeletal contours
and natural S-line
that had been hidden underneath.
The thighs, in particular,
are one of the most common concerns
among women,
as achieving harmony between
the front, side, and back contours
is essential.
For individuals with
a naturally higher body fat percentage,
Liposuction can address
volumes and contours
that are often difficult to improve
through exercise alone.
During weight loss efforts,
many people find themselves thinking,
“Maybe I just need to try harder,”
only to become trapped
in a cycle of fatigue
and weight regain.
Liposuction, however,
offers an opportunity to step away
from this endless dieting loop,
making “maintenance”
a more realistic goal.
By maintaining a stable weight
and continuing to build muscle tone
and skin elasticity after surgery,
patients can preserve
a healthier and more attractive figure
for many years.
The patient presented today
had consistently maintained
a routine of weight training.
However,
despite her overall physique,
she had a relatively high proportion
of Subcutaneous Fat
in the thighs,
and visited Lesarts
seeking improvement.
Rather than emphasizing
a particular contour,
her goal was to achieve
an overall refinement,
creating legs that appeared
slimmer, leaner,
and more balanced.
Preoperative Body Contour Assessment for Thigh Liposuction
[Patient Information]
Age: Late 20s
Height: 169 cm
Weight: 59 kg
BMI: 20.7 kg/m²
Body Fat Percentage: 31.5%
Even patients with similar body measurements
may experience different surgical outcomes.
[Preoperative Assessment for Thigh Liposuction]
The patient worked at
a beauty salon
and had consistently engaged
in weight training.
She had previously undergone surgery
for Thyroid Cancer
and was receiving hormone therapy.
Although her condition
was currently well controlled,
we explained that
the thyroid gland plays
an important role in metabolism,
and that decreased thyroid function
can contribute to weight gain.
For this reason,
we advised her to continue
receiving regular follow-up care.
Ultrasound evaluation prior to
Thigh Liposuction revealed:
Outer thigh:
33/27 mm
Inner thigh:
16/15 mm
Anterior thigh:
17/17 mm
Posterior thigh:
12/13 mm
Knee:
11/12 mm
Overall,
fat suitable for liposuction
was distributed throughout
the thighs.
In particular,
the lateral thighs
and Saddlebag region
projected more prominently,
which contributed to
an appearance of legs
that looked thicker
and relatively shorter.

[Liposuction Design for a Slimmer Leg Contour]
The patient had
a relatively high body fat percentage
throughout the thighs,
with an adequate amount
of removable Subcutaneous Fat.
Therefore,
significant contour refinement
and size reduction
were anticipated after surgery.
Since the most prominent concern
was the lateral thigh contour,
we explained that
maximizing improvement
in this area would be
the primary goal.
However,
the patient had previously undergone
fat harvesting from the posterior thighs
during a forehead fat grafting procedure.
As a result,
surgery in this region
could be technically more challenging
than in other areas,
with the potential for
more pronounced bruising,
swelling, and discomfort.
We also explained that
a certain degree of tissue firmness
might still be palpable,
even after fat removal.
In addition,
if all of the fat
between the thighs and buttocks
were removed,
prominent muscle contours
could become visible,
making the legs appear
shorter and thicker.
To maintain a long
and slender appearance,
a small amount of fat
was intentionally preserved
in this transition zone,
allowing the thighs
and buttocks to connect smoothly.
The patient also had
pre-existing asymmetry,
with differences in fat volume
between the two sides.
Although every effort
would be made
to improve symmetry,
we explained that
complete correction
might not be achievable.
For the inner thighs,
removing all of the fat
could create a depression
in the mid-thigh region,
making the legs appear shorter
and accentuating adjacent prominences.
To achieve the slim,
straight leg contour
desired by the patient,
a subtle amount of fat
was intentionally preserved.
For the anterior thighs,
we planned to remove
as much fat as possible,
without creating
a specific contour.
However,
if there were significant differences
in muscle size
between the right and left thighs,
volume asymmetry
could become more noticeable.
Because differences in thickness
are often more apparent
than contour asymmetry,
a small amount of fat
was preserved
on the side with less muscle bulk
to improve overall balance.

Before-and-After Comparison Following Thigh Liposuction – Dr. Yongseung Seo | Lesarts Plastic Surgery
To create the slim
and slender leg contour
desired by the patient,
the maximum amount of fat
was removed,
while carefully preserving
the overall balance
of the lower body.
As a result,
the lateral contour,
which had appeared bulky
due to prominent Saddlebags,
and the thick,
uneven inner thigh contour,
became straighter
and smoother,
creating the appearance
of legs that looked
longer and slimmer.

Before-and-After Comparison in Patients with Lower Body Obesity – Dr. Yongseung Seo | Lesarts Plastic Surgery
Because the anterior thigh
has a naturally prominent
muscular contour,
it was not possible
to achieve the same straight,
linear appearance
seen in the inner
and outer thighs.
However,
compared with the preoperative state,
a much smoother
and gentler contour
was achieved.
Thigh Liposuction for Asymmetric Legs – Dr. Yongseung Seo | Lesarts Plastic Surgery
Contrary to our preoperative concerns,
the overall asymmetry
was corrected to a degree
that was not noticeable
to the naked eye,
resulting in an excellent
sense of balance.
Many people attempting
long-term weight loss
eventually become discouraged
when they reach a plateau.
Rather than becoming exhausted
and frustrated by the limits
of continued effort,
choosing a more effective solution
to permanently address
body contour concerns
may be a wiser approach.
Liposuction is not simply
a procedure that removes fat.
It can also become
a meaningful investment,
helping patients regain
confidence in their appearance
and rediscover a greater sense
of vitality in daily life.
If you have spent years
hesitating to wear
the clothes you truly want
because of persistent concerns
about thigh size,
perhaps it is time
to leave those worries behind
and meet a new version
of yourself.
That final 2%
you have been striving for—
the journey toward achieving
your ideal slim silhouette—
is one that Lesarts
will accompany safely
from beginning to end.
