Thigh Liposuction-Why Only My Thighs? Correcting a Lower Body Contour Changed After a 7 kg Weight Gain

This is one of the questions
I hear most often during consultations.
Many patients have a relatively slim upper body,
yet notice that fat accumulates predominantly
in the lower body, causing them to feel
that their overall proportions
have become unbalanced.
Changes in body contour following weight gain
are often influenced less by the number
on the scale, and more by
Fat Distribution and the continuity
of body lines.
In this case, we will examine
how an imbalanced lower-body contour
centered around the thighs was evaluated,
and how balance was restored through
a combination of Liposuction
and Fat Grafting, based on
an actual patient case.
Patient Information
Female, late 20s
157 cm / 58 kg
Primary Concern:
Prominent Saddlebags and a desire
to achieve a straighter thigh contour.
Preoperative Body Contour Assessment for Thigh Liposuction and Hip Fat Grafting – Lesarts Plastic Surgery
There were three major concerns
identified before surgery.
First, prominent Saddlebags
that protruded outward
when wearing underwear.
Second, contact between
the inner thighs,
which disrupted the overall
leg contour.
Third, a thigh contour
that projected anteriorly
when viewed from the side.
In particular,
the hip area appeared deficient,
while the saddlebags below
were prominent.
As a result,
the transition between
the upper and lower body
appeared less harmonious,
creating a silhouette that looked
visually interrupted.
Accordingly,
the treatment plan focused
not only on reducing
a specific area of the thighs,
but also on performing
comprehensive Thigh Liposuction,
refining the contour
around the hips,
and supplementing
the deficient hip area
with Fat Grafting.

1. Changes in the Frontal Contour
Before Thigh Liposuction,
prominent Saddlebags
projected outward,
making the thighs appear wider
than the pelvis.
The inner thighs were also in contact,
resulting in no visible gap
between the legs.
After surgery,
the saddlebags were refined,
allowing the transition
between the pelvis
and thighs to appear
more natural.
Reduction of the inner thigh fat
created separation between the legs,
forming a slimmer
and more balanced
lower-body contour.
2. Changes While Wearing Underwear
Following Thigh Liposuction,
the saddlebags were reduced,
while Hip Fat Grafting restored volume
to the upper lateral hips.
As a result,
the contour around the underwear line
became smoother,
and the curve extending from
the waist to the hips and thighs
appeared more harmonious and continuous.

3. Changes in the Side Profile
Before Thigh Liposuction,
the anterior thighs projected forward,
creating a blunt silhouette
in which the thighs extended farther
than the pelvis.
After surgery,
the volume of the anterior thighs
was reduced, allowing the contour
to appear more refined.
As a result,
the knee contour became relatively
more defined,
and the legs appeared
longer and slimmer.

4. Changes in the Hip and Pelvic Contour
Before Hip Fat Grafting,
the pelvic area appeared deficient,
while the saddlebags below projected outward,
creating a silhouette in which
the upper and lower body
appeared visually disconnected.
However,
following Fat Grafting,
the depressed pelvic contour
was restored,
enhancing lateral hip volume
and emphasizing a more natural
S-shaped curve extending
from the waist to the thighs.
5. Changes in Overall Body Balance
Before surgery,
the lower body appeared
disproportionately prominent
compared with the upper body,
creating an impression that
the center of gravity
was shifted downward.
After Thigh Liposuction,
the excess lower-body volume
was refined,
allowing the upper and lower body
to appear more balanced,
while creating a lighter
and more harmonious silhouette.
Thigh Liposuction
is not simply a procedure
to reduce circumference.
Rather,
it is a process aimed at restoring
body balance and contour continuity.
Even at the same body weight,
individuals can appear to have
completely different body shapes,
depending on where fat
is distributed.
For this reason,
careful analysis of each patient’s
natural proportions,
followed by a customized
surgical design,
is essential.
If your lower body appears
