However, even after significant weight loss
and months of strict dieting,
many women continue to struggle with
the stubborn back bulge
and localized fat around the shoulder blades.
These areas often remain unchanged
despite tremendous effort and discipline.
Attempting extreme diets shortly before
the wedding ceremony can also become problematic.
Rapid weight loss may reduce skin elasticity,
weaken the immune system,
and leave you looking tired and depleted
on the very day you want
to look your absolute best.
When asked which body area
causes the most concern before a wedding,
many patients point to their back view.
In particular, the protruding fat
along the back of the arms
and the rounded fullness surrounding
the scapular region are common concerns.
Although these areas are less noticeable
when viewed from the front,
they become highly visible in photographs,
backless wedding dresses,
and from the perspective of guests.

The patient featured in today’s case
already maintained a slim physique
and a healthy body weight.
However, with her wedding day approaching,
she carried a concern that

Despite managing her weight successfully
through a demanding and irregular work schedule,
the localized fat around her

remained resistant to change.
Because these areas play a critical role

[Preoperative Upper Body Assessment]
The patient’s primary concern
was the localized fullness surrounding
the scapular area that protruded
above the bra line.
She also hoped to achieve
a smooth and straight arm-to-shoulder contour
without visible bulging.
During the physical examination,
the arms themselves did not appear
particularly thick overall.
However, fat was concentrated
along the posterior arm,
creating a rounded projection toward
the back and outer arm region.
As a result, the arms and upper body
appeared larger and heavier
than their actual size.
The scapular region and
the area around the underarms
did not contain excessive fat deposits.
Nevertheless, sufficient localized fat
was present to allow meaningful
improvement through liposuction contouring,
particularly for refining the silhouette
visible when wearing a dress.

Posterior arm: 9 / 10 mm
Scapular area: 9 / 12 mm
Anterior arm: 4 / 5 mm
Deltoid muscle thickness: 17 / 16 mm
Based on these measurements,
a dramatic reduction in arm volume
was considered unlikely.
Because the patient’s existing fat layer
was relatively thin,
the degree of size reduction
would naturally be limited.
We also explained that achieving
a perfectly straight right-angle arm line
beginning directly from the shoulder
would be challenging.
The patient’s underlying muscle structure
and soft-tissue characteristics
placed natural limitations on
the final contour that could be achieved.

[Customized Planning for a Bride-to-Be]
Although achieving a perfect
right-angle shoulder line
would be difficult in this case,
the patient naturally had
a relatively low upper-body muscle mass.
Because of this, we determined that
arm and back liposuction
could successfully create the slim,
straight silhouette she desired.
Since the patient already had
a relatively small amount of body fat,
an overly aggressive approach
could have increased recovery time
and created unnecessary stress
before the wedding ceremony.
For this reason, we focused on
the areas most visible in a wedding dress,
prioritizing a safe recovery while maximizing
the visual improvement of the overall silhouette.
The scapular area was identified as
the patient’s greatest source of concern
and one of the most important regions
for shaping the back and side profile
when wearing a wedding dress.
Because this area responds well to
liposuction contouring and carries
a relatively low risk of contour irregularities,
we planned a more active fat reduction
to create a smoother and more refined back line.
The anterior arm region contained
very little subcutaneous fat.
We explained that attempting
excessive fat removal in this area
would provide minimal contour improvement
while potentially increasing the risk
of surface irregularities after surgery.
For this reason, conservative treatment
was considered the safer option.
The underarm area does not usually
have a major impact on the appearance
of a wedding dress silhouette.
However, because it had been
a long-standing concern for the patient,
we decided to include this area
as part of the overall treatment plan.

Photographs were taken approximately
one month and two weeks after surgery
under identical lighting and environmental conditions.
