thigh swelling and accumulating thigh fat

Thigh liposuction
can be one effective solution
for these concerns.

However, there is one important point
to remember.

Liposuction is not a surgery
aimed at reducing body weight.

When weight increases,
thigh fat is often the first to accumulate,
yet when losing weight,
it is usually the last area to slim down.

By removing stubborn thigh fat
quickly and aesthetically,

The patient we introduce today

Preoperative Assessment for Thigh Liposuction

• Age: Late 20s
• Height: 158 cm
• Weight: 54.2 kg (previously maintained 47–50 kg)
• BMI: 21.7 kg/m²
• Body Fat Percentage: 31.3%

[Preoperative Body Analysis and Patient Concerns]

“My inner thighs rub and feel uncomfortable.”

As her weight increased,
the patient experienced noticeable thigh thickening,
with significant volume increase in the inner thighs,
causing daily discomfort.

On visual examination,
the inner thighs showed heavy fat accumulation,
with no visible gap from the knees upward
when the legs were brought together.

The outer thighs (saddlebags area)
also had substantial fat deposits,
making the legs appear thicker and shorter.

Ultrasound measurements revealed:

• Saddlebags: 39/47 mm
• Inner thigh: 16/18 mm
• Anterior thigh: 11/13 mm
• Knee area: 12/11 mm

Although the most prominent fat
was in the inner and outer thighs,
there was also visible accumulation
in the anterior thigh and knee areas.

1. Inner Thighs

As this was the patient’s primary concern,
we planned to remove as much fat as safely possible.

However, we explained that
depending on each individual’s pelvic structure
and muscle volume,
achieving a completely separated thigh gap
may not always be realistic.

To maintain a smooth, natural contour,
a small amount of fat
along the muscle boundary areas
was intentionally preserved.

Because the inner thigh
is also one of the most common areas
for skin laxity,
we informed the patient in advance
that when fat volume is significant,
wrinkling along the groin line
may occur after surgery.

2. Saddlebags Area

From the outer knee
to the most protruding saddlebags region,
fat was removed as thoroughly as safely possible.

However, at the junction
between the thigh and buttock,
complete fat removal can create
an overly sharp boundary,
making the legs appear shorter
and the buttocks more projected.

Therefore, similar to the inner thigh,
a controlled amount of fat
was preserved
to maintain a smooth, natural transition.

3. Buttock Sagging Concern

The patient was concerned
about potential buttock sagging.

In general, sagging is influenced by
how much of the total posterior thigh thickness
is occupied by the gluteal crease.

To prevent elongation of this crease,
the supportive structures
must not be damaged.

Additionally, the fat
directly beneath the buttock
helps provide structural support,
so excessive removal in this area
must be avoided.

We emphasized that
special care would be taken
during surgery
to preserve these critical structures.

However, even if the gluteal crease
does not lengthen,
when the thigh circumference decreases
after thigh liposuction,
the relative proportion
of the crease within the overall thickness
may increase.

As a result,
a slightly more sagging appearance
can occur visually,
which was also explained in advance.

Before thigh liposuction,
there was almost no gap
between the thighs,
and the prominent saddlebags
made the legs appear
shorter and thicker.

After surgery,
a visible thigh gap was created,
and the lines became smoother
and more refined—
making the legs appear longer.

The prominent saddlebags were
cleanly refined
and now connect naturally
to the hips,

creating a smoother transition
between the thigh and hip line.

As the widest outer point
shifted slightly upward,
the legs appear
longer and slimmer.

The gluteal crease showed minimal change.
As expected before surgery,
a reduction in thigh circumference
slightly increased
the proportional visibility
of the crease.

However, since there was
no preoperative hip sagging,
this subtle difference
was not clinically significant.

Following thigh liposuction,
the patient also began
a structured diet program.
Her weight decreased to 51.5 kg,
and her body fat percentage
reduced to 28.1%.

As mentioned earlier,
liposuction itself
does not significantly lower body weight.
However, it meaningfully impacts
body fat percentage,
which is more closely related
to aesthetic contour.
In this case,
body fat decreased by 3.2%.

Liposuction is not
simply fat removal.
It is the beginning
