[Gynecomastia] Will losing weight reduce the chest? Understanding gynecomastia and personalized treatment

Hello, this is Dr. Jihoon Moon
from Lesarts Plastic Surgery.

“Will my chest
reduce if I lose weight?”

This is one of
the most common questions
in the consultation room.

And my answer
is always the same.

“It might,
or it might not.”

Gynecomastia and dieting – Dr. Jihoon Moon, Lesarts Plastic Surgery

This may sound
a bit ambiguous,
but there is a reason.

Gynecomastia
cannot be explained
by body weight alone.

The prevalence
among Korean men
ranges from 40% to 60%.

While this has increased
with rising obesity rates,
that is not the only factor.

Many people think
gynecomastia is simply
fat accumulation in the chest.

This is not entirely wrong,
but it is not
fully accurate either.

The male chest
consists of fat tissue,
glandular tissue,

and supporting
connective tissue.

The proportion
and structure
of these components

determine
the overall
chest contour.

The key point
is that this composition
varies between individuals.

Even with the same
body weight
and body fat percentage,

some individuals
have a flat chest,
while others show prominence.

Causes of gynecomastia – Dr. Jihoon Moon, Lesarts Plastic Surgery

These differences
are not simply
about fat quantity.

They are determined
by fat thickness,
presence of glandular tissue,

and the structure
of connective tissue
in the chest area.

On ultrasound,
these differences
become more evident.

In men without gynecomastia,
glandular tissue
is usually not visible.

In contrast,
patients with gynecomastia
show clear glandular tissue

beneath
the nipple
and areola.

This difference
is structural,
not related to weight.

Clinically,
some men
have a narrow waist

and a more
feminine body contour.

Intuitively,
this may suggest
more glandular tissue.

However,
many of these cases
actually show minimal presence.

Some individuals
also present
softer features

in appearance,
overall impression,
and even personality traits.

However, this does not mean
they have
hormonal abnormalities.

Nor does it indicate
an underlying
congenital condition.

In most cases,
test results
remain within normal ranges.

This area
is better understood
as individual variation,

rather than
a defined
medical disorder.

Differences in
fat distribution,
connective tissue arrangement,

and glandular expression
create variations
in body shape.

These differences
may be considered
subtle genetic variations,

similar to
minor changes
at a molecular level.

Therefore, gynecomastia
is not a single condition,
but a broad spectrum.

It ranges from
fat-dominant types
to glandular-dominant forms,

to cases with
dense connective tissue,
or mixed patterns.

Another important factor
in gynecomastia
is fat thickness.

Even with the same fat,
a uniformly distributed layer
and a localized thick deposit

can appear
completely different
in contour.

A thicker fat layer
in the chest area
is often resistant

to weight loss
and exercise.

As a result,
even after fat reduction,
the chest may appear unchanged.

In some cases,
as surrounding fat decreases,
the chest becomes more prominent.

Treatment of gynecomastia – Lesarts Plastic Surgery

Ultimately, gynecomastia
is not a single condition,
but a broad spectrum.

Therefore, treatment
cannot rely
on a single approach.

The extent of
fat reduction,
glandular removal,

and management
of connective tissue
must all vary.

The key to treatment
is not simple removal,
but structural reconstruction.

Now, returning
to the original question.

Will it disappear
with weight loss?

Gynecomastia surgery and dieting – Dr. Jihoon Moon, Lesarts Plastic Surgery

This is partially true,
but in many cases,
it is not.

The difference
is determined
by structure, not weight.

Ultimately, gynecomastia
is not simply
a weight-related issue.

It is the result
of individual
structural differences.

If the structure differs,
the treatment approach
must also change.

Gynecomastia surgery
does not have
a single fixed solution.

It requires
a personalized approach
for each patient.
