[Thigh Liposuction] How Much Can Camel Hump-Like Saddlebags Be Improved with Liposuction?
Hello, this is Dr. Pyongrim Choi
from Lesarts Plastic Surgery.
Recently, during lower body liposuction consultations,
I have noticed a growing number
of patients presenting with
so-called “Western-style lower body contours,”
where the pelvic area
appears excessively prominent
compared to before.
They often appear slim
in the upper body and face,
yet gain weight primarily
in the lower body.
When fat accumulation occurs
mainly in specific body areas,
there can be several contributing factors.
One possible cause
is pelvic imbalance,
which may lead
to swelling and waste accumulation,
making it easier
for fat to build up
in the lower body
and creating imbalance
between the upper and lower body.
Another common reason
is lower body obesity
caused by insufficient physical activity,
combined with
high-calorie Westernized eating habits
and excessive intake
of processed foods.
In these cases,
patients may maintain
a relatively slim upper body
and waistline,
while the area extending
from the hips and pelvis
to the thighs
becomes disproportionately fuller,
resulting in noticeable
body balance concerns.
A body type with excessively developed saddlebags — Lesarts Plastic Surgery
The main issue occurs
when excessive fat accumulation develops
along the outer thigh area,
commonly referred to as
the saddlebags region.
When this area becomes overly prominent,
it can make the overall body shape
appear heavier and wider,
while also causing
the leg line to look
shorter and less refined.
In cases like this,
thigh liposuction can improve
not only aesthetic concerns,
but also functional discomfort as well.
Let’s take a look
through an actual patient case.

Patient concern:
Wanted to improve
the hump-like protruding saddlebags,
while also naturally correcting
overall lower body obesity.

pre-operative photos
before thigh liposuction surgery.
[Pre-operative diagnosis before
thigh liposuction]
Overall,
the patient showed
a lower-body dominant body type.
In particular,
the saddlebags area
was excessively protruded.
The inner thigh area,
especially the upper portion,
also appeared prominently projected overall.
Along the inner knee
(Chanel line area),
fat accumulation was observed
together with bony prominence.
Additional contour correction
around the hip line
and buttock area
was also considered necessary.
Skin elasticity
had decreased due to
repeated weight fluctuations.
[Planned improvement direction through
thigh liposuction]
The surgical plan
for saddlebags liposuction
was designed to maintain
smooth continuity
between the upper and lower areas,
while minimizing
wrinkling and skin sagging.
(The hump-like protrusion
in the saddlebags area
showed no evidence
of tumors or other lesions
on ultrasound examination,
confirming that it was suitable
for correction through liposuction.)
Full thigh liposuction
was planned
to reduce inner thigh friction
and minimize protrusion
of the anterior thigh contour.
Additional hip line liposuction
was also planned
to reduce overall volume
around the hips and buttocks.
The procedure was designed
using a 3-way incision approach,
including:
-Hip line incision
-Lower buttock incision
-Outer anterior thigh incision
(The 3-way approach helps achieve
more uniform fat reduction
in the saddlebags area,
while allowing
greater size reduction
with smoother
and more natural contour transitions.)

These are the patient’s
before-and-after photos
taken 5 months after
thigh liposuction surgery.
The protruding outer thigh contour
was refined inward,
creating a smoother
and slimmer side silhouette.
In addition,
the previously disconnected transition
between the pelvis
and thigh contour
was naturally blended together.
As a result,
the thighs appear visually longer,
with improved overall body proportions.
The entire lower body silhouette
also appears noticeably slimmer
and more balanced overall.

The most important point
in this thigh liposuction case
was determining
how much fat should be removed
to achieve a natural result.
Approximately 7000cc of fat
was removed in total
through thigh
and hip line liposuction,
with nearly 3000cc
coming specifically
from the saddlebags
and surrounding hip area.
This clearly demonstrates
how much excess fat
had accumulated
around the saddlebags region,
and explains why
the visible transformation
was so significant.
However,
would removing even more fat
have created a slimmer appearance?
The answer is no.
Overcorrection
would likely have caused
more severe wrinkling,
along with increased skin sagging,
resulting in a look
far less natural
than what the patient desired.
In addition,
through this thigh liposuction surgery,
the patient achieved
not only aesthetic improvement,
but also functional improvement.
Friction between the inner
and outer thighs during walking
was reduced,
movement during exercise
became more comfortable
and natural,
and persistent pressure
while wearing clothing
also decreased.
In other words,
the surgery improved
not only the body contour,
but also overall daily comfort,
leading to
even greater patient satisfaction.
Saddlebags liposuction
is not simply about
removing as much fat as possible.
Good results are achieved only when
balanced fat reduction,
uniform thickness preservation,
and full-body contour design
are all carefully harmonized together.
For patients concerned
about prominently protruding saddlebags,
I hope liposuction can help improve
not only aesthetic appearance,
but also overall functional comfort
in daily life.
