Thigh Liposuction: Achieving a Smoother, More Confident Leg Contour
Hello, this is Dr. Yongseung Seo
from Lesarts Plastic Surgery.
If you spend most of your day
sitting at an office desk,
your legs can often feel
more swollen and heavier.
Because of this,
many people follow strict diets
while pushing themselves
through Pilates
and various forms of exercise,
even when they are already
physically exhausted.
And for those who are married
and raising children,
that effort often becomes
two or even three times greater.
Unfortunately,
not everyone is rewarded
for that hard work.
This is because some areas
of the body are naturally resistant
to diet and exercise.
Two of the most common examples
are the inner thighs
and saddlebags.
Trying to reduce fat
in these areas
through willpower alone
can be extremely difficult.
Life’s unavoidable circumstances
and the limits
of physical endurance
often stand in the way.
Eventually,
many people become discouraged,
regain the weight,
and find themselves trapped
in a cycle of repeated setbacks.
Today’s case involves a patient
who put in tremendous effort
but ultimately visited our clinic
to finally address
the stubborn thigh fat
that refused to go away.
[Patient Information]
Age: Mid-30s
Height: 169 cm

Body Fat Percentage: 29.4%
*Even patients with similar body measurements
may experience different outcomes
following liposuction surgery.
[Preoperative Body Assessment for Thigh Liposuction]
Preoperative Body Assessment for Thigh Liposuction – Lesarts Plastic Surgery
The patient was married
and had a history of childbirth.
She worked in an office setting
and consistently attended
Pilates classes
despite her busy schedule.
However,
even with her dedication
to exercise,
her thighs remained noticeably larger
than her upper body.
In particular,
the inner thighs
and saddlebags
were highly developed,
giving her lower body
an appearance similar to that
of an athlete.
As seen in the preoperative photos,
the inner thighs
were in constant contact,
creating a crowded appearance
with no visible thigh gap.
The prominent saddlebags
also made the legs appear
shorter and thicker
than desired.
In addition,
the front of the thighs
projected outward significantly,
making the thighs appear
especially bulky
when viewed from the side.
Ultrasound measurements showed
substantial subcutaneous fat deposits:
Outer thighs: 34 / 32 mm
Inner thighs: 19 / 19 mm
Front thighs: 17 / 18 mm
Back thighs: 13 / 14 mm
Inner knees: 12 / 10 mm
Based on these findings,
we expected the patient
to achieve a favorable outcome
from thigh liposuction.
[Designing the Ideal Thigh Liposuction Plan]
The patient’s primary concerns
were the inner thighs
and outer thighs,
and her goal was
to achieve a smooth,
naturally contoured leg line.
Key Design Principles for Thigh Liposuction
Our approach was to remove
as much excess fat as possible
throughout the thighs,
while intentionally preserving fat
in specific areas
that contribute
to a smooth
and balanced contour.
For example,
in the inner thighs,
the region extending
from above the knee
to the mid-thigh,
as well as the transition area
between the thigh and buttocks,
requires careful preservation.
Removing all of the fat
from these regions
can create an unnatural break
or indentation in the contour.
Particular attention was given
to the area beneath
the buttocks.
Not only would excessive fat removal
compromise the appearance,
but this fat also provides support
for the buttocks.
If removed completely,
the buttocks may lose support
and begin to sag.
For this reason,
fat in this area
was intentionally preserved.
However,
because the patient had
a relatively large amount of fat
and reduced skin elasticity
along the inner thighs,
we explained in advance
that some degree
of skin laxity
could develop following surgery.
The upper portions
of the outer thighs
and front thighs
were also treated conservatively.
If too much fat is removed,
the underlying muscle contours
can become visible,
creating hollowed areas.
Therefore,
a small amount of fat
was intentionally left behind
to maintain smooth
and natural transitions.

With the removal of fat
from the inner thighs,
the patient developed
a visible thigh gap
that was completely absent
before surgery.
As a result,
the legs appeared
much lighter,
more refined,
and visually longer.
However,
as discussed before surgery,
a small degree of skin laxity
was observed along
the upper inner thigh
and groin area.
On the outer thighs,
the reduction of
the saddlebags
created a smooth,
continuous curve
from the knees
to the pelvis,
contributing to a slimmer
and longer-looking leg contour.

The front thighs
were also refined
through targeted fat removal.
The previously rounded
and protruding contour
was transformed into
a softer,
more gradual curve,
making the legs appear
straighter,
slimmer,
and more balanced overall.
Balancing office work,
household responsibilities,
childcare,
and regular exercise
is not easy.
For many patients,
liposuction
represents more than
a simple surgical procedure.
It is the final step
that reveals the results
of years of hard work
that were hidden beneath
stubborn fat deposits.
The changes ahead
will not only improve
how your workout clothes fit,
but may also transform
the confidence
you carry into everyday life.
For the moment
when your efforts finally shine,
Lesarts Plastic Surgery
will be with you,
paying attention
to every last detail.
Are you ready
to complete your journey
toward a healthier,
more confident version
of yourself?
