To determine the cause of the patient’s symptoms,
we first performed an ultrasound examination.

The ultrasound demonstrated
well-developed glandular tissue in both breasts,
and a palpable glandular mass was also confirmed
during the physical examination.

Based on the degree of
breast enlargement and skin laxity,
the patient was diagnosed with
true gynecomastia (Simon Grade 2A).

A noticeable prominence was present
along the central portion of the chest,
and mild bilateral inverted nipples
were also observed.

However, the nipple inversion
was not severe enough to require
a separate corrective procedure.

Because the enlarged glandular tissue
was pulling the nipples inward,
we anticipated that removing the gland
would likely improve the nipple appearance naturally.

The patient also reported
no pain or tenderness when
the glandular tissue was compressed.

Based on these findings,
we recommended gynecomastia surgery consisting of
gland excision and lateral chest liposuction
to reduce chest volume and create
a flatter, more masculine chest contour.

At the 2-week postoperative follow-up,
the prominent projection that had previously
been visible in the center of the chest
has already been noticeably reduced.

The combination of gland excision and
chest liposuction has visibly decreased
the overall chest volume, creating
a flatter and more masculine chest contour.

As the fullness of the anterior chest
has been successfully refined,
the chest now demonstrates
a more natural masculine appearance.

The appearance of the nipples also appeared
stable during this early stage of recovery.

However, we explained that
additional follow-up would be important
after the swelling subsides and
postoperative tissue firmness gradually resolves,
allowing the final nipple contour
to be evaluated more accurately.
