Teenage Gynecomastia,
hormonal changes and treatment timing
at Lesarts Plastic Surgery

Gynecomastia that develops
during puberty
may appear temporarily
as part of the growth process.
However, in some cases,
the chest enlargement persists
without significant improvement,
eventually leading
to surgical treatment.

The main cause of
teenage gynecomastia
is hormonal fluctuation
during adolescence.
Temporary imbalance
between sex hormones
can result in
glandular tissue overgrowth.

Even when treatment is desired,
adolescent gynecomastia
usually requires
a 6-month observation period
to determine whether
the condition improves naturally.

If gynecomastia that began
during adolescence
continues for several years,
or shows no improvement
despite exercise and weight control,
along with symptoms such as
nipple protrusion,
a palpable lump, or pain,
it is recommended
to seek medical evaluation
and establish an appropriate
treatment plan.

This patient had experienced
gynecomastia symptoms
since adolescence.

Now in his 20s,
the condition had not improved
even after reaching adulthood,
which led him to visit the clinic
for surgical treatment.

Ultrasound examination confirmed
overgrown glandular tissue,
and the condition was diagnosed as
true gynecomastia, Simon Grade 2A.

To reduce the overall
chest size,
removal of the
glandular tissue
was essential.
In addition,
the surrounding chest fat
that contributed to
the enlarged appearance
also required treatment.

If only the gland
had been removed,
the size reduction
would have been limited
mainly around the nipple area,
which could result in
an unnatural chest contour
or visible indentation.

For this reason,
gland excision and
chest liposuction
were planned together
to create a smoother
and more balanced result.

Additionally,
to prevent irregular depression,
indentation, or tissue adhesion
caused by the empty space
after gland removal,
anti-adhesion treatment and
tissue repositioning
were included in the surgical plan.

Through this approach,
the goal was not only
to reduce the central protrusion
caused by gynecomastia,
but also to create
a naturally flat chest contour
with smooth transitions
from the upper chest
to the lateral chest line.

Let’s review the
3-month post-operative results
to observe the change
in chest size and contour.

The enlarged chest size
and protruding contour
that had been a concern
since adolescence
showed clear improvement
through gynecomastia surgery.

In particular,
the previously swollen
areolar area and
projecting nipple shape
were also effectively corrected.

With the combination of
liposuction and
tissue repositioning,
the surgery achieved more than
simple protrusion correction.
A more defined and masculine
upper chest contour
was also created,
leading to high patient satisfaction.

Since adolescence,
the patient had experienced
increased chest size,
along with protruding
areola and nipple shape,
which caused discomfort
in daily life.

One of the biggest concerns
commonly experienced by
gynecomastia patients
is clothing restriction,
and this patient also wished
to resolve that issue.

Before surgery,
the most embarrassing concern
when wearing T-shirts
was the noticeable protrusion
in the center of the chest.
After gland removal and
chest liposuction,
the protrusion improved significantly,
creating a firmer and
more masculine chest contour.
A visible improvement
in posture was also observed.

By completely addressing
the underlying cause
of the long-standing
gynecomastia,
the patient is expected
to achieve even better results
with continued exercise
and proper lifestyle management.
