Thigh liposuction-Sculpting straight legs for tall body types: preventing sagging and seamless contour
Hello, this is Dr. Pyongrim Choi
from Lesarts Plastic Surgery.
Recently,
the proportion
of tall women
has been increasing.
As a result,
even with normal weight,
more patients
visit the clinic
with concerns
about their thigh line.
In particular,
protrusion in the
outer thighs (saddlebags)
and inner thighs
often prevents
the legs from appearing straight.
This can make
the legs look
shorter than they are.
Today, we will review
a case of a tall patient
in her late 20s.
We will explore
how thigh liposuction
can create straight legs,
and how detailed design
plays a key role
in achieving refined results.
1. Anatomical sculpting for straight thighs
Removing more fat
does not automatically
create straight legs.
It requires understanding
the muscle and bone structure,
and refining fat evenly.
1) Key of the outer line
Deep fat
in the saddlebag area
must be reduced.
However,
over-removal
can expose bone contours.
A smooth curve
from pelvis to knee
must be preserved.
2) Completion of the inner line
From the most prominent
inner thigh area
to above the knee,
fat must be
evenly refined.
Over-removal
in this area
can cause sagging
or surface irregularities,
so precise control
is essential.
3) Overall proportion
If the upper thigh
is wider
than the lower thigh,
sufficient removal
in the upper area
is needed,
while maintaining
a natural gradient
toward the knee.
This creates
a visually straight
leg silhouette.
2. Detail defines the final result
Even when
a straight line
is achieved,
details determine
the overall quality
of the outcome.
1) Why preserve the banana roll
It acts
as a structural support
to prevent sagging.
Excessive removal
below the gluteal fold
can lead to
loss of support
and downward
displacement of the hip.
This may create
an additional crease,
known as a double fold.
Preserving this layer
is a key
surgical detail.
2) Connecting saddlebags to the hip line
Refining only the saddlebag area
is not sufficient.
A smooth transition
to the outer hip must be created.
This prevents step-offs
between regions, and creates
a visual lifting effect
of the hips.
The result is
longer-looking legs
and a lifted silhouette.
3)Knee area refinement (“Chanel line”)
If inner thigh suction
is not connected
to the knee, a bulky knee
appearance may remain.
Creating a smooth slope
from mid-thigh to the knee
is essential for a true
straight leg line.
[Patient case]
Female, late 20s
172 cm / 63.8 kg / BMI 21.6
Goal: smooth thigh contour
with a slim, straight line
Upper thighs
appear more
laterally spread
compared to
the lower thighs.
The inner thighs
are generally
in contact,
with fat
more concentrated
toward the upper area.
From the side view,
the anterior thigh
appears protruded
due to
muscle and fat.
The hip shows
slight sagging,
with poor connection
to the
saddlebag area.

Front view: overall refined
into a straight line
The inner thigh is more refined in the upper area,
creating a natural gradual contour
toward the lower thigh.
In particular, the inner knee
has been refined,
enhancing the connection
to the calf line.

Side view: fat over the
anterior thigh muscle
was removed, reducing the
protruding appearance.
The contour appears smooth
and natural, without excessive
muscle definition.

Posterior view: the area below the hip
and the saddlebag region
are smoothly connected,
creating a natural transition.
Visually, the starting point
of the hip appears higher,
resulting in a relative lifting effect
without fat grafting.

Posterior view: removal of
saddlebag fat lifts the outer contour,
creating a visually longer leg line.
Both inner and outer lines
connect smoothly to the knee and calf,
without abrupt indentations.
The gluteal fold
remains stable,
without deepening
or elongation,
as support
below the hip
is preserved
to minimize
sagging.
People often say
to see the forest,
not just the trees.
But in aesthetics,
both the forest
and the details matter.
While many aim
for a straight
thigh line,
simply creating
a uniform shape
is not the answer.
Factors such as
muscle structure,
bone shape,
and skin elasticity
must all be considered
for natural results.
Attention to
these subtle details
leads to higher
satisfaction
and more refined
outcomes.

your body accurately
and guide optimal results.
