The patient in this case had
a lower body–dominant figure
and hoped to achieve a smoother leg contour
through thigh liposuction.

Let’s examine how her body changed
by comparing the before-and-after photos below.

According to her body shape analysis,
this patient presented with the following characteristics:

1.She had low skin elasticity
and minimal muscle mass,
with a fat-dominant lower body type
and a high amount of localized fat accumulation.

2.She had noticeable hip dips,
which caused the starting point of her legs
to appear lower, affecting overall proportions.

3.She also showed signs of buttock sagging,
resulting in a visible infragluteal fold
(crease beneath the buttocks).

Based on this evaluation,
we proposed the following thigh liposuction plan:

1.Perform thigh liposuction
to remove concentrated fat in the thighs,
creating a smoother, more defined leg contour.

2.To add volume and curve to the hips,
we recommended hip dip fat transfer,
using the patient’s own harvested fat
to fill the sunken hip area
and elevate the transition between hip and thigh.

3.To prevent further buttock sagging
and maintain the natural under-buttock line,
we planned to leave a small amount of fat
beneath the buttocks during hip line liposuction.
This reduced heaviness while maintaining support
for the lower gluteal area.

The patient underwent thigh liposuction
combined with hip line liposuction
to remove stubborn fat from the lower body
and achieve a smoother, more feminine leg contour.

Although she did not proceed with
hip dip fat transfer,
which could have further enhanced the curves,
her 6-month post-op results show clear improvements
in both thigh circumference reduction
and the formation of a visible thigh gap.

To avoid buttock sagging
and preserve the infragluteal fold,
a portion of the fat beneath the buttocks was intentionally retained.
This approach balanced volume removal
with supportive shaping during hip line liposuction.

Due to her reduced skin elasticity,
the patient was instructed to wear a compression garment
for over three months—longer than average—
to prevent skin laxity
and help the skin adhere smoothly
to the reduced fat volume.

As with all patients,
we emphasized that even with satisfying surgical results,
long-term maintenance requires
consistent exercise,
a balanced diet,
and a focus on healthy lifestyle habits
to prevent rebound and preserve results.
