The case introduced today
involves a patient
with a naturally slim build.

She previously maintained
a weight in the high 40kg range,
but recently experienced weight gain.

She visited the clinic
due to increased arm volume
and contour concerns.

The patient also had
a history of breast
and accessory breast surgery.

Most importantly,
she strongly desired
a slim, straight arm line

without visible
muscle prominence
or bulkiness.

Age: Early 20s
Height: 162 cm
Weight: 51.1 kg
BMI: 19.5 kg/m²
Body fat: 26.7%

Ultrasound findings showed
posterior arm: 11/12 mm
anterior arm: 6/5 mm

Shoulder muscle thickness
was measured at
21/16 mm.

Overall fat volume
was not excessive,
but several challenges existed.

The posterior bulge area,
which concerned the patient most,
contained both fat and fibrosis.

While fat removal
could create improvement,
fibrotic tissue was significant.

The right shoulder muscle
was noticeably larger
than the left side.

This indicated
clear asymmetry
in muscle and skeletal structure.

The anterior arm
had minimal fat,
with volume mainly from muscle.

This was a typical case
where arm thickness
is determined by muscle rather than fat.

Although the patient
desired a “bone arm” look,
her arms were muscle-dominant.

A significant portion
of the volume
was from muscle, not fat.

Therefore, even with
maximum fat removal,
extreme slimness was not achievable.

Instead, we explained
that strategic fat preservation
can create a slimmer visual effect.

In other words,
if fat is removed
from all key areas,

such as above the elbow,
between muscle groups,
inner hollow lines, and upper elbow side,

the natural contours
of muscle and bone
become overly exposed.

This can make the arms
appear thicker visually,
and disrupt overall proportions.

Additionally, fibrotic tissue
cannot be effectively removed
through suction alone.

Although volume decreases
after arm liposuction,
a firm texture may still remain.

We explained that
fibrotic tissue increases
surgical difficulty.

It may prolong
procedure time
and recovery duration.

Postoperative swelling,
bruising, and discomfort
may also be more pronounced.

Lastly, in this case,
the right-side muscle
was significantly larger.

Even after surgery,
this area could still
appear relatively thicker.

We discussed two options,
accepting asymmetry
with maximum fat removal,

or preserving volume
on the left side
to balance proportions.

After thorough consultation,
the patient chose
maximum fat removal.

The surgical plan
was designed to prioritize
fat reduction despite asymmetry.
