Some men struggle with concerns related
to the size and shape of the areola.

Normally, the male areola and nipple
are expected to be small and flat.
However, when glandular tissue develops broadly
beneath the nipple, the areola can be
pushed outward and appear enlarged.

This overgrowth of glandular tissue not only
enlarges the chest but also causes the nipples
and areolae to protrude, making the chest
look even bigger and wider from the outside.

Because these changes result from structural
issues in the chest’s subcutaneous layer,
if discomfort is felt due to such alterations,
it is important to understand that
accurate diagnosis and proper treatment
are necessary.

This patient began noticing chest enlargement
on both sides during adolescence,
and even into adulthood, regardless of
weight changes, the chest remained
noticeably prominent, causing significant
psychological stress.

In particular, the silhouette of the chest
was emphasized under clothing, and the
bulging around the nipples and areola
was a constant source of concern,
leading to increasing discomfort in
daily life.

More recently, he also began to feel
a firm lump beneath the chest,
which added to his discomfort.
For this reason, he visited our clinic
seeking an accurate diagnosis
and an appropriate treatment plan.

During consultation, we examined
the patient’s chest and found that both sides
showed rounded protrusion centered on the nipples,
clearly visible to the eye.

On palpation, firm glandular tissue masses
were detected beneath both sides of the chest.

The areola area appeared wide and swollen,
and the overall chest size showed
a distinct difference compared to
a typical male chest.

Although skin laxity was not severe,
excess fat in the lower and lateral chest
made the protrusion appear even more pronounced.

An ultrasound examination confirmed
glandular tissue overgrowth on both sides.
Based on these findings, the patient was diagnosed
with true gynecomastia, Simon Grade 2B.

Accordingly, the following surgical plan
was recommended for correction:

1.Glandular Tissue Excision
Through a periareolar incision, the enlarged tissue
would be removed to resolve the root cause
of chest protrusion and areolar swelling.

2.Chest Liposuction
Liposuction of the lower and lateral chest
would be performed together, refining the chest volume
and achieving a flatter, natural contour.

3.Adhesion & Depression Prevention
Surgical techniques would be applied
to prevent tissue adhesion or nipple depression,
ensuring an even, stable chest shape.

Below, we will review the comparison photos
taken at 1 month after gynecomastia surgery
to see how the chest has transformed.

When comparing the before-and-after results
of gynecomastia surgery, the chest that was
rounded and protruded around the nipples
has been corrected.

The unnecessary volume was removed, leaving
the upper body line much flatter and cleaner.

Additionally, the previously swollen nipples
and areolae
were improved into a natural shape,
and the overall silhouette from chest to abdomen
was refined into a masculine and smooth contour.

When comparing the before-and-after photos
in a thin short-sleeve T-shirt, we can see that
the protrusion of the chest has been
completely corrected.

Before gynecomastia surgery, the chest enlargement
from gland overgrowth, along with the swollen nipples
and areolae
, created an unnatural silhouette,
making the T-shirt fit look bulky and uncomfortable.

At 1 month post-op, with gland removal and
chest liposuction combined, the chest now appears
flat and toned — similar to the look of
well-trained chest muscles under a T-shirt.

However, since this patient had long-standing gynecomastia
since adolescence
, stabilization of the surrounding tissues
and recovery of skin elasticity require
careful follow-up for at least 3–6 months,
sometimes up to a year
.

At the 1-month stage, recovery is progressing well,
but full stabilization of the chest contour
will take more time.

We emphasized to the patient the importance of
consistent garment compression, weight management,
exercise habits, and regular follow-up visits

to maintain and enhance the results.

With proper post-op care, the final outcome
is expected to become even more refined
than the current result.
