In this post,
we would like to introduce
a patient who underwent
thigh liposuction surgery to improve
concerns regarding an uneven leg contour
caused by fullness involving
the inner thighs and
the saddlebag regions.

The patient hoped to address
the appearance of legs that seemed
shorter than their actual skeletal proportions
and to reduce the heavy,
less refined impression of
the lower body through surgery.

During consultation,
the patient expressed a desire
to maximize refinement of
the inner thigh fullness
in order to achieve
a visible thigh gap and
a straighter, more streamlined
leg contour.

In addition,
the patient hoped to reduce
the rounded projection of
the outer saddlebag areas,
which contributed to the perception
of shorter legs, thereby creating
a smoother lateral contour and
a more proportionate lower body silhouette.

Following a comprehensive evaluation,
the patient’s thigh contour demonstrated
the following characteristics:

-Increased fullness involving
the inner thighs

-Prominence of the saddlebag region,
which projected higher than
the level of the pelvis

-The transition extending from
the saddlebag area toward
the posterior thigh was identified
as an important aesthetic consideration.

As a result,
the inner thighs appeared
crowded and closely apposed,
while the elevated saddlebag contour
contributed to the impression of
shorter and heavier-looking legs.

the Inner Thigh and Saddlebag Contours]

For patients with this body type,
the surgical strategy focuses on
maximizing refinement of
the inner thigh fullness
within a safe and appropriate range
to create a clearer thigh gap
and a straighter leg contour.

However, excessively aggressive removal of
the saddlebag region may disrupt
the smooth transition between
the lower buttock and upper thigh,
potentially producing an unnatural contour
or accentuating volume deficiency
beneath the buttocks.

Therefore, rather than pursuing
maximum fat extraction alone,
the procedure was designed to
strategically contour the saddlebag area
while preserving a harmonious connection
between the hips, buttocks,
and posterior thighs, thereby achieving
a visually longer, slimmer, and
more proportionate lower body silhouette.

1. Changes in the Front and Posterior Views Following Thigh Liposuction Surgery

Before thigh liposuction surgery,
the upper inner thighs were
closely apposed to one another,
while the central portion remained separated,
creating an irregular contour along
the inner thigh line.

Liposuction was therefore performed
from the upper inner thighs
extending toward the knees,
with the goal of creating
the straightest contour possible
within the patient’s anatomical limits.

As a result,
a visible thigh gap was created,
and a slimmer, straighter,
and more elegant leg silhouette
was achieved.

In addition,
the saddlebag region projected outward
beyond the level of the pelvis,
increasing the apparent width
of the lower body and causing
the perceived starting point
of the legs to appear lower
than the actual pelvic position.

Because saddlebag liposuction involves
an area that transitions into
the lower hip contour and
the posterior thigh beneath
the buttocks, careful attention
must be paid to maintaining
a smooth and harmonious connection.

For this reason,
the amount of fat removed
was strategically adjusted while
continuously evaluating the contour line,
rather than pursuing maximal extraction.

As a result,
the protruding saddlebag fullness
was naturally improved,
while the transition throughout
the entire thigh contour
became smoother and more balanced.

The legs also appear
visually longer,
while the overall thigh width
has been reduced, creating
a more refined and straight-leg appearance.

2. Changes in Thigh Circumference Following Thigh Liposuction Surgery

The reduction in thigh circumference
following thigh liposuction surgery
can be appreciated most clearly
from the side profile.

The fullness previously present
along the anterior thigh
has been effectively improved,
while the irregular contour of
the posterior thigh has also
been refined into a smoother
and more streamlined appearance.

Through strategic contouring of
the saddlebag region and
the posterior thigh,
it was possible to achieve
a lighter and more balanced
side profile without creating
the appearance of buttock ptosis
or loss of lower buttock support.

Although the primary goal
in this patient was to improve
the contours of the inner thighs
and saddlebag areas,
the procedure focused not simply
on reducing localized volume.

Instead, careful attention was paid
to creating smooth transitions
between adjacent anatomical regions,
allowing the patient to achieve
the thigh contour she had hoped for.

This case highlights that
successful thigh liposuction involves
more than removing excess fat alone.

By considering each patient’s
skeletal proportions, fat distribution,
and aesthetic goals,
it is possible to create
natural-looking improvements that remain
harmonious from every angle
and during all movements,
which is one of the most
important principles of body contouring surgery.
