Many patients become concerned
about their chest size
while working out at the gym
or during situations such as showering.

Because many of these patients
have exercised consistently,
they often already recognize
that the chest enlargement
is not always related
to simple weight gain.

During consultation and diagnosis,
patients commonly present with
increased chest size,
a palpable lump beneath the nipple,
swollen areolar tissue,
and in some cases,
even chest pain.

These are all representative symptoms
of gynecomastia caused by glandular overgrowth.
If the discomfort is significant,
it is important to establish
an appropriate treatment plan
through accurate diagnosis.

During consultation,
it is essential to evaluate
when the symptoms first appeared
and how they progressed over time.
Since self-diagnosis
or visual assessment alone
has limited accuracy,
ultrasound examination and
proper tissue evaluation
are considered essential.

This patient had experienced
a small palpable lump
beneath the nipple area
and gradual chest changes
since his teenage years.

However, because there was
no significant pain
and the chest enlargement
did not rapidly progress,
the condition was not initially
considered a major concern.

Despite consistent exercise,
including push-ups
and cardio workouts,
the overall chest shape
showed little improvement,
and the protruding contour
became increasingly noticeable.

In addition,
a firm lump around
the nipple area
continued to be palpable,
leading the patient
to seek a proper
gynecomastia evaluation
for accurate diagnosis
and treatment planning.

During the consultation,
a firm palpable lump
was identified beneath
the nipple area
where the patient
had experienced discomfort.

Ultrasound examination
confirmed mild
glandular tissue overgrowth,
and the condition was diagnosed as
Simon Grade 1 gynecomastia.

Although the condition
did not qualify for
insurance coverage,
the patient strongly desired
correction,
and surgery was performed.

[20s, Grade 1, Gynecomastia Surgical Plan]

1. Gland excision
– Removal of the palpable
glandular tissue
beneath the nipple
to correct the uneven
chest protrusion

2. Chest liposuction
– Removal of localized chest fat
that contributed not only
to protrusion
but also to imbalance
in the overall chest contour

3. Tissue repositioning and anti-adhesion treatment
– Performed to help
the chest skin settle smoothly
after surgery

In addition,
upper chest contour reshaping
through tissue repositioning
was included to maximize
the overall gynecomastia correction effect.

Let’s now review
the 3-month post-operative results
to observe the changes
before and after surgery.

Although the patient had
Simon Grade 1 gynecomastia,
he experienced discomfort
from the palpable chest lump
and a chest contour
that did not improve
through exercise alone.
For this reason,
he chose to undergo
gynecomastia surgery.

After surgery,
the previously uneven
and protruding chest contour
visible from both
the front and side views
became naturally flatter
and more refined.

In particular,
through the additional
upper chest contour reshaping,
the chest itself developed
a firmer and more athletic appearance,
similar to the look achieved
through consistent workouts.

The patient also expressed
high satisfaction,
stating that the
overall upper body proportion
appeared much more balanced
and refined than before.

When wearing clothes,
the most noticeable change
was the improvement in
nipple protrusion
and the transition to
a more natural fit.

Because the patient already had
a slim body type
and exercised consistently,
simply correcting
the chest contour issue
completely transformed
the overall appearance.

Although the chest size itself
was classified as
Simon Grade 1 gynecomastia,
this case demonstrates that
even relatively mild gynecomastia
can cause significant discomfort
due to the influence of
glandular tissue.

The surgery not only improved
the external appearance,
but also provided
greater comfort
in daily life
through symptom relief.
