The forehead is one of the most important
features influencing a person’s
overall first impression.
When the forehead begins with
a flat contour from the hairline or
has a naturally receding profile,
the upper face may appear less
three-dimensional, causing the overall
facial contour to look relatively flat
and lacking in vitality.
These anatomical characteristics can occur
regardless of age or body type, and
when the central forehead is congenitally deficient in volume,
they are often difficult to camouflage
with makeup alone.
Most notably,
a depression within the central forehead
can make the brow ridge appear
relatively more prominent, while also
creating the impression that the
lower face projects further forward.
As a result, the facial profile may appear

The patient presented in this case
shared these anatomical characteristics.
Seeking a softer, more harmonious facial appearance,
she underwent forehead fat grafting
to restore natural volume and improve
the overall balance of her upper facial contour.
Before undergoing forehead fat grafting,
the patient demonstrated insufficient forehead volume,
causing the forehead to appear relatively
narrow and creating a flat upper facial contour.
Rather than beginning with a smooth,
natural curve that transitions from
the forehead to the brow and nasal bridge,
the region between the hairline and
the eyebrows appeared slightly recessed.
As a result, the brow ridge appeared
relatively more prominent, contributing to
a stronger and more angular facial profile.
To address these concerns,
the treatment plan focused not on
dramatically changing the patient’s
overall facial shape, but rather on
strategically restoring volume to the
areas where it was deficient.
By enhancing the central forehead contour,
the goal was to create a more three-dimensional,
refined, and naturally balanced forehead profile,
while preserving the patient’s unique

The procedure utilized autologous fat harvested from the thighs,
which provided a reliable source of healthy fat
for forehead augmentation.
During the primary fat grafting procedure,
the focus was on gently restoring volume
throughout the forehead rather than
overcorrecting the contour with
a large amount of transferred fat.
Because the survival rate of fat grafts
varies from patient to patient,
a staged treatment plan was adopted.
Following an appropriate recovery period,
the degree of fat retention,
the overall contour, and the natural flow

Only if additional refinement was required
was a secondary forehead fat grafting procedure
performed, allowing the final contour
to be completed in a gradual and
more predictable manner.

Following forehead fat grafting,
the patient’s forehead developed
a smooth, natural curvature, creating

Before surgery,
the central forehead appeared depressed,
while the forehead sloped backward
toward the hairline, giving the upper face
a relatively narrow and flat appearance.
Because of the insufficient forehead volume,
the brow ridge appeared relatively more prominent,
contributing to a stronger and
more angular facial impression.
Following forehead fat grafting,
these structural characteristics were naturally corrected,
restoring soft, three-dimensional volume
to the central forehead.
As a result,
the patient’s facial appearance became
noticeably softer, more refined, and more youthful,
with a smoother and better-balanced
upper facial contour.
The patient also expressed
a high level of satisfaction
with the surgical outcome.
Rather than creating excessive fullness,
the procedure focused on precisely restoring volume
only where it was needed, allowing the
upper facial contour to achieve
a natural, stable, and harmonious appearance
while improving the overall balance of the face.
