even stepping outside can feel exhausting these days.

As temperatures rise, clothing naturally becomes
lighter and thinner.

However, for men who feel self-conscious about their chest,
summer can actually make going out more stressful.

For those with gynecomastia, even a thin T-shirt
may make the chest contour look more prominent.

Some men end up wearing multiple layers despite the heat
or even hesitate to go outside.

At the same time, some patients postpone seeking treatment
for years because of concerns that gynecomastia surgery
may significantly disrupt their daily life.

In this case, we will look at the changes seen
12 days after gynecomastia surgery
in a male patient in his 40s who visited our clinic
with these concerns.

Upper Body Evaluation Before Gynecomastia Surgery – Lesarts Plastic Surgery

The patient had been concerned about
his protruding chest since his teenage years.

At that time, awareness of gynecomastia
was not as widespread as it is today
,
so he did not seek medical evaluation.

However, concerns about his chest protrusion
continued over the years.

After beginning medication for male pattern hair loss
in his 30s, he felt that his chest
had become larger than before
.

Some medications used to treat male pattern hair loss
may cause or worsen gynecomastia.

We explained this possible association,
and the patient also suspected a connection
between his medication and chest changes.

Even after changing his hair loss medication
and consistently managing his weight through exercise
,
there was no significant improvement
in the chest protrusion.

He therefore visited our clinic
to determine whether he had gynecomastia
and to discuss appropriate treatment.

The patient was 181 cm tall
and weighed 90 kg.

On physical examination, there was no significant
accumulation of fat in the chest area

compared with his overall body fat level.

Diagnosis for Gynecomastia Surgery – Lesarts Plastic Surgery

The pectoralis major muscles and upper chest contour
showed an overall masculine appearance
.

However, moderate to significant enlargement
was observed mainly in the lower pole
.

In particular, the lower chest around the nipple
and areola protruded in a rounded shape
,
making the gynecomastia more noticeable.

There was also mild asymmetry
between the left and right sides
.

The right side was slightly larger
than the left.

This degree of asymmetry is relatively common,
and perfectly symmetrical chests

Ultrasound examination showed overgrowth of glandular tissue
concentrated around the areola

The glandular tissue was relatively well developed
compared with the thickness of the surrounding fat.

This was one reason why weight loss and exercise alone
did not significantly improve the chest protrusion

Based on the physical examination and ultrasound findings,
the patient was diagnosed with true gynecomastia, Simon grade IIa.

To correct the protruding chest contour,
we planned a combination of glandular tissue removal
and liposuction
