Male gynecomastia can occur
transiently during puberty as part of
normal hormonal changes.

For this reason, many adolescents and
their families tend to regard
chest enlargement during the teenage years
as a temporary condition and
often postpone seeking medical evaluation.

However, if gynecomastia symptoms
that first appeared during adolescence
persist into the patient’s 20s without
showing signs of improvement, or if
diet, weight reduction, and exercise
fail to produce meaningful changes,
a proper medical assessment becomes important
to determine the underlying cause.

Particularly, if additional symptoms such as
a palpable glandular mass,
breast tenderness, or
nipple discharge are present,
a more proactive treatment plan
may be necessary to alleviate
the physical and psychological discomfort
associated with the condition.

The patient presented in this post
first noticed symptoms of gynecomastia
during his teenage years.

Despite waiting for spontaneous resolution,
the enlarged chest contour remained unchanged,
and the symptoms persisted into his 20s,
ultimately leading him to seek
a more definitive evaluation and treatment.

[Patient Information and Diagnostic Findings]

Age: Early 20s
Height / Weight: 181 cm / 81 kg
Diagnosis: Mixed Gynecomastia, Simon Grade 2A
Skin Elasticity: Good
Glandular Thickness: 27 mm (Right) / 32 mm (Left)

On visual examination,
the patient demonstrated a noticeably
enlarged and protruding chest contour,
with visible puffiness involving
the areolar region bilaterally.

Based on these findings,
the condition was classified as
Simon Grade 2A gynecomastia,
and ultrasound evaluation confirmed that
the primary cause of the patient’s
gynecomastia symptoms was
glandular hypertrophy.

Upon palpation,
firm glandular tissue was readily detected
beneath the areolae, presenting as
a palpable mass-like structure.

The patient was already aware of
these glandular nodules and reported
that they had been one of
the longstanding sources of discomfort
in daily life.

In addition to glandular enlargement,
the surrounding subcutaneous fat layer
was also contributing to
the increased chest volume.

Accordingly, it was explained that
the most appropriate surgical approach
would involve simultaneous gland excision,
chest liposuction, and tissue repositioning
,
allowing both the excess volume and
structural deformity to be corrected while
helping the chest settle into
a smoother, flatter, and more
naturally masculine contour.

The following photographs were obtained
on the day of suture removal following
gynecomastia surgery
.

The previously protruding chest contour
has been effectively corrected,
revealing a smoother and more
naturally contoured thoracic silhouette.

The areolae, which had appeared
enlarged and puffy due to
underlying glandular hypertrophy,
have also decreased in prominence,
while the projected appearance
has been noticeably improved.

In addition,
because chest liposuction and
tissue repositioning were performed
simultaneously with gland excision,
the transition of the upper body
appears smooth and harmonious,
without visible depressions, contour irregularities,
or an operated appearance.

Even though these photographs were taken
during the early recovery period,
the chest contour already demonstrates
a flatter, more masculine, and
well-balanced appearance, suggesting that
further refinement can be expected
as residual swelling and tissue firmness
continue to improve over the coming months.

From the side profile,
the improvement in the contour
that had previously appeared somewhat awkward
due to the prominent projection
of the chest relative to
the upper abdomen can be
appreciated more clearly.

Following gynecomastia surgery,
the protrusion associated with
glandular enlargement has been corrected,
while the palpable mass that had
been a persistent source of concern
for the patient has also been eliminated.

As a result,
the thoracic contour now appears
firmer, smoother, and more naturally masculine,
demonstrating a well-balanced transition
from the upper chest to
the upper abdomen.

Although this stage of recovery is
commonly associated with residual swelling
and BioBond-related tissue firmness,
these postoperative changes are not
readily noticeable from an external appearance.

The patient also reported experiencing
minimal discomfort in daily life,
stating that routine activities have
been resumed without significant limitation,
which further contributed to his
overall satisfaction with the early
post-operative outcome.

Particularly, one of the patient’s
greatest concerns had been
the chest contour that appeared
noticeably prominent and unnatural
whenever clothing was worn.

Following gynecomastia surgery,
this issue improved almost immediately,
allowing the patient to enjoy
greater freedom in clothing choices
regardless of the season.

When gynecomastia symptoms begin
during adolescence or early adulthood,
especially when accompanied by
a palpable glandular mass,
patients often realize that
their condition extends beyond
simple fat accumulation.

At that point, concerns regarding
treatment itself, uncertainty about
which specialty to consult, and
difficulty choosing an appropriate clinic
can understandably become major sources
of stress and hesitation.

At Lesarts Plastic Surgery,
we strive to provide a comfortable
and discreet treatment experience through
one-on-one private consultations
with male surgeons
and
individual recovery rooms designed
to maximize patient privacy and convenience.

In addition, our approach focuses
not only on removing the underlying
cause of gynecomastia, but also on
applying plastic surgical principles
and contouring techniques
to create
an aesthetically pleasing and naturally
masculine chest shape.

If you have been struggling with
gynecomastia symptoms and their impact
on your confidence and daily life,
we encourage you to visit
