Some assume that chest enlargement
is simply the result of
weight gain and fat accumulation,
while others believe it is merely
a temporary change associated with
adolescence and delay seeking care.

However, gynecomastia is
a relatively common condition,
and surgical treatment can provide
an immediate reduction in chest volume
while allowing most patients
to resume daily activities
without significant difficulty.

For this reason,
individuals hoping to improve
their symptoms are generally advised
not to delay evaluation
and treatment unnecessarily.

In particular,
when gynecomastia symptoms
that first develop during adolescence
are left untreated for many years,
the weight of the enlarged chest
may gradually contribute to
skin laxity through the effects
of gravity and aging.

In addition,
long-standing chest enlargement
can cause the overlying skin
to stretch over time,
increasing the likelihood of
wrinkling or residual skin laxity
even after gynecomastia surgery.

The patient discussed in this post
is a man in his 30s
who had experienced
gynecomastia symptoms since adolescence.

Despite regular exercise
and ongoing weight management,
his symptoms remained unchanged,
eventually causing discomfort
in daily life and leading him
to seek surgical treatment
at our clinic.

[Patient Information and Diagnostic Findings]

Age: Early 30s
Height / Weight: 175 cm / 84 kg
Diagnosis: Mixed Gynecomastia, Simon Grade 2A
Skin Elasticity: Moderate

On visual examination,
the patient’s chest demonstrated
prominent puffiness centered around
the areolar region, with
both sides showing noticeable enlargement
and anterior projection.

Based on these findings,
the condition was classified as
Simon Grade 2A gynecomastia,
and ultrasound examination confirmed
the presence of hypertrophied
glandular tissue beneath
both areolae.

In addition,
subcutaneous fat deposits
were also contributing to
the increased chest volume,
indicating that both glandular tissue
and adipose tissue played
a role in the deformity.

Accordingly, it was explained that
optimal correction would require
simultaneous removal of
both the glandular tissue
and excess fat, while
tissue repositioning would also be necessary
to allow the chest contour
to settle into a more natural
and masculine shape.

Through the following
before-and-after gynecomastia photographs,
we will review the patient’s
healing progress at 2 months post-operatively
and evaluate the improvement achieved
after more than a decade
of persistent gynecomastia symptoms.

The enlarged and protruding chest
that had previously resulted from
gynecomastia symptoms has been
effectively refined, creating
a flatter and more gradual
chest contour.

The areolae, which had appeared
puffy due to glandular overgrowth,
have also decreased in prominence,
demonstrating a more natural
size and shape.

In addition,
chest liposuction contributed to
improvement of the contour beneath
the lower and lateral chest,
eliminating the shadowing that had
accentuated the fullness of the chest.

As a result,
the previously noticeable volume
has been significantly reduced,
allowing the upper body to appear
more balanced, masculine,
and naturally contoured.

From the side profile,
the previously prominent transition
between the lower chest and
upper abdomen has become
significantly smoother and
more gradual in appearance.

As the protrusion caused by
gynecomastia was corrected
through surgery, the contour of
the anterior upper body has been
restored to a more natural
and masculine silhouette.

Although the patient demonstrated
a moderate degree of skin elasticity
prior to surgery, recovery has
progressed favorably without the development
of noticeable wrinkling, skin laxity,
or other contour-related concerns.

At 2 months post-operatively,
the chest contour already appears
stable and well-settled, suggesting
that further improvement may be expected
as residual swelling and tissue firmness
continue to resolve over time.

The patient shared that
one of the greatest sources of stress
in daily life had been
the limitations he experienced
when choosing clothing for work,
social activities, and everyday outings.

He explained that, despite wearing
a compression garment during
the early recovery period,
being able to comfortably wear
a thin T-shirt alone
was the most meaningful
and satisfying change following
gynecomastia surgery.

The patient is currently
approximately 2 months post-operatively,
a stage at which
compression garments are generally
no longer required.

Although mild residual swelling
is still present,
he reported that being able
to wear any style of clothing
without hesitation and feeling comfortable
in environments requiring exposure
of the chest, such as saunas
or swimming pools, has greatly improved
his confidence and overall quality of life.

As a result, he expressed
a very high level of satisfaction
with the surgical outcome.

When gynecomastia symptoms develop
during adolescence,
a period in which physical appearance
often carries significant importance,
the psychological burden can be substantial.

If these symptoms persist
into adulthood, the associated stress
may become even more pronounced,
while self-imposed limitations
in daily activities and clothing choices
can gradually expand over time.

Most importantly, obtaining
an accurate diagnosis and identifying
the appropriate treatment options
without unnecessary delay
are essential steps in achieving
meaningful symptom improvement.

Through this process,
establishing a suitable treatment timeline
and undergoing timely intervention
can help patients not only improve
their physical symptoms,
but also regain confidence
and greater freedom in their daily lives.
