is not always

The patient in
today’s case
was a business owner
with a busy lifestyle.

Over the past
two years,
she experienced
approximately 10 kg
of weight gain.

As a result,
her concern
about the
lower body contour
gradually increased.

She had previously
undergone procedures
for the arms
and abdomen,
reflecting strong interest
in self-care.

Age: Late 30s

Height: 165 cm

Weight: 63.3 kg

BMI: 23.3 kg/m²

Body Fat Percentage: 29.2%

Even with similar
body measurements,
results of liposuction surgery
may vary depending
on individual conditions.

Preoperative Body Assessment for Thigh Liposuction and Pelvic Fat Grafting

Overall, the patient
did not have
excessively high
total body fat
for her frame.

However, fat
was concentrated
around the outer thighs
and inner thighs,
affecting leg contour.

The saddlebags area
protruded outward,
making the legs
appear thicker
and slightly shorter.

Fat accumulation
along the inner thighs
also created
a closed appearance
with both thighs
touching together.

Mild asymmetry
between the legs
was also observed
during evaluation.

On ultrasound examination,
fat thickness was
measured in
several thigh regions
for surgical planning.

Saddlebags: 29 / 25 mm

Inner thighs: 23 / 19 mm

Posterior thighs: 11 / 12 mm

Anterior thighs: 16 / 18 mm

Inner knee: 9 / 10 mm

The degree of
fat asymmetry
was not considered
severe overall.

Instead, the difference
appeared more related
to muscle asymmetry
between both sides.

Therefore, the surgical plan
focused on carefully
adjusting the amount
of fat removal
to improve symmetry.

However, the patient
presented with
very pronounced
hip dips
around the pelvis.

In the greater trochanter area,
significant tissue adhesions
were also observed
during preoperative
clinical evaluation.

Because of this,
extensive and careful
tissue release
was required
during surgery.

The patient was informed
that creating
a completely smooth
hip contour
might not always be possible.

This limitation can occur
even with
pelvic fat grafting,
depending on
the degree of adhesions.

Surgical Design for Thigh Liposuction and Pelvic Fat Grafting

For the saddlebags,
fat removal was planned
as thoroughly as
safely possible
during liposuction.

However, due to
the severe
hip dip contour,
careful balance
was essential.

If excessive fat
between the hip
and thigh
were removed,
the contour could worsen.

This could make
the hip dip line
appear more irregular
and visually shorten
the leg length.

Therefore, the goal
was to create
a balanced
and proportionate
hip–thigh transition.

For the inner thighs
and inner knees,
fat removal was planned
more aggressively
to reduce bulk.

At the same time,
the mid-thigh area
was carefully refined
to maintain
a smooth straight leg line.

For the anterior thighs,
significant muscle asymmetry
was already present
before surgery.

Because of this,
removing all fat
equally on both sides
was not appropriate.

Instead, thickness adjustment
was necessary
to achieve
better overall
leg symmetry.

The patient was informed
that slightly more
fat might remain
on one side
for balance.

For the fat grafting,
the patient desired
a fuller and
rounder hip shape.

However, due to
severe adhesions
in the hip dip region,
volume retention
might be limited.

If the procedure
were performed
too aggressively,
the contour could
appear more uneven.

In such cases,
the hip dip area
may appear deeper
rather than smoother.

Therefore, the final
extent of fat grafting
was planned
to be determined
during surgery.

three months after surgery
under identical
lighting conditions.

1. Outer Thigh

The previously prominent
saddlebags
have been significantly
reduced after
liposuction surgery.

As a result,
the contour now
extends smoothly
toward the pelvic line
without outward bulging.

This change
creates a more
slender and
straight leg silhouette
compared to before.

2. Inner Thigh

A large amount
of fat was removed
from the inner thighs
to reduce
overall bulkiness.

However, because
the patient had
relatively strong
thigh muscles,
a full thigh gap
was not expected.

Even so,
compared to before,
a noticeable space
between the thighs
has formed.

This improvement
greatly reduced
the previously
crowded appearance
and uneven contours.

3. Pelvic Contour

As anticipated before
the hip fat grafting surgery,
the severe adhesions
in the hip dip area
limited upward lifting.

Because of this,
fat grafting was
performed carefully
to match the
existing contour level.

Although the patient
did not achieve
a fully rounded
gluteal shape,
the previously sunken
hip line improved
significantly.

The hollow contour
along the hip dip region
was noticeably
smoothed and
more balanced.

In this thigh liposuction case,
unnecessary fat layers
above well-developed
muscle contours
were selectively removed.

The procedure focused
on maintaining proper
symmetry and balance
throughout the
entire lower body line.

As a result,
a slimmer and
more refined
lower body silhouette
was achieved.

Although this is
only the 3-month stage,
the previously concerning
hip–thigh transition line
already appears smoother.

As recovery progresses,
the contour will
continue to refine
with time and
natural tissue settling.

The fit of
tight activewear
or denim jeans
is also expected
to look more flattering.
