Among men experiencing gynecomastia symptoms,
those with higher body weight often first try
exercise or weight loss
to reduce chest size.

This is because, in most cases,
the cause of an enlarged chest
is assumed to be fat accumulation.

However, if there is no change in chest size
even after losing a certain amount of weight,
it becomes difficult to attribute the condition
solely to obesity-related body changes.

Through the gynecomastia surgery case
introduced in this post,
we will explain this in greater detail.

When examining the patient’s chest shape,
both sides showed a clear increase in size
and a prominent protruding contour.

Due to nipple and areola enlargement,
the chest appeared noticeably swollen.

On palpation, firm lumps were detected,
raising suspicion of glandular tissue development.

To confirm this accurately,
an ultrasound examination was performed,
which revealed hyperplastic glandular tissue.

The patient was diagnosed with
true gynecomastia,
corresponding to Simon grade 2A.

The primary cause of chest enlargement
was identified as glandular overgrowth.

However, because there was also
a significant amount of fat
in the lower and lateral chest areas,

it was determined that
glandular excision combined with
chest liposuction surgery
was necessary.

1. Glandular tissue excision
The incision was made
along the areolar margin
to minimize visible scarring
after recovery.

By removing the glandular tissue
that directly contributed to
chest enlargement and
areola–nipple protrusion,
the overall chest contour
was corrected.

2. Chest liposuction
Fat from the lower chest
and lateral chest areas
was aspirated
to eliminate additional causes
of chest volume.

This allowed the contour
from the chest to the flanks
and upper abdomen

to connect smoothly
and naturally.

3. Upper chest contouring (pec contouring)
After removing both
glandular tissue and fat,
tissue repositioning was performed
to prevent chest depression
or a flat appearance.

The surgical plan aimed
to create a firm, athletic-looking
upper chest line
,
similar to the result
of consistent exercise.

Below, through photos taken
2 months after gynecomastia surgery,
we will review the treatment outcome
and improvement in
gynecomastia symptoms.

Before gynecomastia surgery,
the patient’s chest on both sides
was noticeably enlarged,
with clear protrusion.

Around the nipples and areola,
visible swelling was present,
creating an appearance
that felt embarrassing.

Because the patient had
a larger build,
these features appeared
even more pronounced.

At 2 months after gynecomastia surgery,
the chest volume was fully reduced,
and the previously expanded nipples
and areola
were corrected.

As a result,
the chest changed into
a flat, clean, and masculine contour.

The before-and-after difference
was even more noticeable
in the t-shirt fit.

Before gynecomastia surgery,
the chest volume was clearly visible
through the t-shirt,
causing ongoing discomfort
in daily life
.

However, two months after surgery,
the chest size was reduced,
and the shape shifted
to a more masculine contour,
making a firm, defined outline
stand out even under a t-shirt.

Because the patient had
a larger body frame,
the gynecomastia symptoms
appeared more pronounced before surgery,
leading to multiple inconveniences
in everyday activities.

At present, the overall chest volume
has decreased in a stable manner,
and with the improved upper-body proportion,
the patient shared that he
has regained confidence.

Although the two-month postoperative stage
still involves mild residual swelling,
the upper chest line already appears
firm, as if shaped through exercise.

As the previously protruding areas
have been refined,
a clean and balanced upper-body line
has now settled.

With continued care going forward,
this patient is expected
to achieve an even more refined appearance,
going beyond the surgical outcome itself.
