Today, we’ll look at a male in his 30s
who underwent gynecomastia surgery
at Lesarts Plastic Surgery.

This patient visited us after a health checkup
showed possible signs of gynecomastia.
As time passed, he noticed that his
chest protrusion and areola enlargement
became increasingly visible.

For an accurate diagnosis and

When evaluating the patient’s condition
to diagnose gynecomastia, we observed
that both sides of the chest had noticeable volume.
During palpation, a firm glandular tissue
was detected beneath the nipples.

The side chest and underarm areas
contained excess fat tissue, making the chest
appear even larger and more pronounced.

To assess the extent of glandular growth,
an ultrasound was performed, clearly confirming
bilateral glandular hyperplasia.

In the accessory breast area, no glandular tissue was found,
but increased fat distribution caused localized enlargement.

Based on these findings, the patient was diagnosed
with True Gynecomastia (Simon Grade 2A).
The goal was to reduce chest volume
and restore a masculine contour.

1. Gland Removal
Through a periareolar incision, the glandular tissue
causing enlarged and protruding chest was excised.
This reduced volume and corrected nipple projection.

2. Liposuction of Lower, Side, and Axillary Chest
Since gland removal alone could not fully reduce size,
liposuction of the lower chest, side chest,
and anterior axillary area was performed
to achieve a balanced chest contour.

3. Tissue Repositioning
After gland removal, tissue was repositioned
to prevent chest depression
and create a smooth, natural chest shape.

Postoperative photos at 2 months show
the visible improvements after surgery.

Before surgery, the nipples were prominently protruding
due to glandular tissue beneath them.
After gland removal, this projection has been fully corrected.

At 2 months postoperative, the chest size is visibly reduced,
and the nipples no longer show sharp protrusion.

In addition, chest liposuction performed simultaneously
reduced volume in the side and lower chest,
creating a more balanced and smooth chest contour.

Before surgery, the patient was most concerned
about chest protrusion visible through T-shirts.
From the side, the chest protruded forward,
and the nipple area volume made the upper body appear awkward.

Through gynecomastia surgery, the chest line was transformed
into a more masculine contour.
The prominent chest and nipple projection above the T-shirt
were corrected, reducing daily discomfort.

This patient underwent both gland removal and liposuction,
achieving a well-balanced chest shape with stability in all directions.
