This patient, a woman in her early 40s,
had been experiencing prolonged lower abdominal protrusion
and skin sagging since childbirth,
which caused continuous discomfort in daily life.

Although her body weight had remained relatively stable,
her lower abdomen appeared noticeably bulging,
and the loss of skin elasticity led to visible sagging,
making her waistline disappear —
her primary source of concern.

She initially believed that the issue could be corrected
with abdominal liposuction alone,
as the lower belly volume seemed to be her main problem.

However, upon detailed examination,
it was revealed that the abdominal protrusion
was not solely due to fat accumulation,
but rather a combination of skin laxity
and a weakened abdominal wall that had shifted outward.

Accordingly, a combined surgical approach was planned,
involving S-type abdominoplasty,
rectus muscle plication,
and abdominal liposuction,
to restore firmness and improve overall contour balance.

Before surgery, the patient’s abdominal condition
showed a clearly concentrated volume below the navel.

In addition, the overall skin elasticity of the abdomen
had weakened, causing both fat and skin to sag downward
under gravity and accumulate beneath the navel,
which led to the formation of visible skin folds.

This downward heaviness not only caused sagging and creasing,
but also made it impossible for the body line
to appear smooth and toned.

To address these issues, the following surgical plan was made:

1.S-Type Abdominoplasty (Umbilical Floating) + Abdominal Wall Plication (Rectus Muscle Repair)
The sagging lower abdominal skin was lifted
to create a firm abdominal contour.
Since the navel position was not significantly low,
the floating technique was applied,
tightening the separated abdominal muscles
from the upper to lower abdomen
to prevent any further outward bulging.

2.Full Abdominal Liposuction
Considering the skin thickness and fat distribution,
liposuction was performed across both
the upper and lower abdomen,
extending to the flanks and lower back
to create a more defined and contoured waistline.

3.Hip Dip (Iliac Area) Fat Grafting
Fat harvested from the abdomen was transferred
to the hip dip areas (130cc each side)
to restore volume balance and
smoothly connect the waist-to-hip transition,
enhancing the overall body proportion and silhouette flow.

Through this combination of abdominoplasty,
abdominal liposuction, and hip dip fat grafting,
the patient achieved a dramatic improvement
in body contour and lower abdominal balance,
as confirmed in the before-and-after photos.

Before surgery, the area below the navel showed skin that had
pushed forward and downward, creating a bulging appearance.

Although it may have seemed like simple fat accumulation,
the underlying issue was actually skin laxity and a weakened abdominal wall,
both of which cannot be corrected through diet or exercise alone.

To address these structural concerns,
S-type abdominoplasty, abdominal wall plication,
and abdominal liposuction were performed together.

As a result, the sagging lower abdomen and the protruding abdominal wall
were successfully corrected, while excess fat was refined,
allowing the waistline to become more defined and cinched.
The overall upper body contour now appears firmer, lighter, and more balanced.

Changes are also clearly visible
when observing the abdominal shape while bending forward.

Before surgery, the abdominal wall bulged outward when bending,
and the skin sagged downward, causing the lower abdomen
to fold and project forward — resulting in visible creases and bulges.

After abdominoplasty, these issues were completely corrected.
The lower abdominal silhouette now remains smooth and firm,
without any unevenness, folds, or loss of elasticity,
maintaining a clean, natural contour even when the body is bent.

From the side view, it’s clear that the downward sagging of the abdomen
caused by gravity has been fully corrected,
