After a cesarean section
or abdominal surgery,
adhesions often develop
as the skin and
subcutaneous tissue
stick together.

When adhesions occur,
the skin surface
becomes uneven
and is pulled inward,
making the abdominal contour
appear less smooth.

In particular, when
abdominal skin sagging
or diastasis recti
is present together,
the overall body contour
may look unstable,
and daily discomfort
can also occur.

For this reason,
many patients visit
the clinic for
professional consultation
to plan abdominal contouring surgery
after sufficient recovery time.

[Patient information]

Age: Late 30s
Height: 165 cm
Weight: 55 kg
BMI: 20

Even with similar physical characteristics,
surgical results may vary
depending on the individual.

In this patient’s case,
after twin delivery
by cesarean section,
adhesions developed
above the surgical scar.

In the lower abdomen,
skin sagging occurred,
and diastasis recti
caused abdominal protrusion,
leading the patient to seek
abdominal contouring consultation.

In front and side views,
sagging was clearly visible
from every angle.

Through palpation, we confirmed
reduced skin elasticity
caused by pregnancy and childbirth,
along with diastasis recti.

These factors were diagnosed
as the main causes
of the patient’s body contour changes.

Especially from the side view,
the abdominal wall protruded forward
from the upper abdomen
down to the lower abdomen.

Due to gravity,
the degree of skin sagging
became more severe
toward the lower abdomen.

When bending forward,
the abdominal wall
failed to provide support,
causing the abdomen
to spill forward visibly.

The skin also appeared
weak and loose,
sliding downward
without resistance.

In addition, adhesions caused
the skin surface
to appear uneven
and bumpy.

Although the patient’s
skin elasticity
was not severely compromised,
significant diastasis recti
prevented firm abdominal support,
making the sagging
appear more pronounced.

To correct these complex abdominal issues,
the following surgical plan
was established.

1. Abdominoplasty
(M-type, umbilical reconstruction)

– Lifts skin sagging
from the upper
to lower abdomen
– Removes excess skin
along with the C-section scar
and existing navel
– Reconstructs and repositions
the navel
into the desired vertical shape

2. Rectus muscle repair

(Abdominal wall plication, triple-lock technique)

– Tightens stretched rectus muscles
that lost elasticity
during pregnancy and childbirth
– Restores abdominal wall support
– Aims for not only
aesthetic improvement,
but full functional recovery

3. Adhesiolysis
(Adhesion release)

– Releases adhesions
around the C-section scar
and surrounding tissue
– Smooths the previously
tethered abdominal surface

4. Abdominal liposuction + hip fat grafting

– Refines the waistline
through abdominal liposuction
– Transfers fat to the hip area
to create a smooth transition
from waist to pelvis
– Improves overall body balance

Below are the 2-month postoperative photos
after abdominoplasty,
showing how the bulging abdomen
and irregular adhesions
caused by two C-sections
were corrected and refined.

[Procedures performed]

Abdominoplasty, umbilical correction,
rectus muscle plication,
abdominal liposuction,
hip fat grafting

After cesarean delivery,
The patient experienced skin sagging
From the upper abdomen
To the lower abdomen,
Along with surgical scars
And skin adhesions.

Rectus diastasis also developed,
Resulting in overall abdominal deformation.
The patient sought treatment
To improve these concerns.

In photos taken 3 months after
Abdominoplasty surgery,
Sagging concentrated in
The lower abdomen
Was clearly corrected.

With combined abdominal liposuction
And hip fat grafting,
A slimmer, more feminine
Upper body contour
Can be clearly seen.

Especially in a bent-forward posture,

Due to rectus diastasis
And decreased abdominal elasticity,

The lower abdomen
Previously pushed forward
As if spilling outward,

Has now been completely corrected.

By plication of the rectus muscles,
Abdominal wall function was restored,

And the severely sagging skin
Was lifted,

Improving the downward,
Flowing appearance of skin laxity.

In addition, preexisting adhesions
Present before abdominoplasty

Were smoothly released,

Allowing the skin surface
To remain even and smooth,

Even in a bent posture,

Resulting in a more stable
And refined abdominal contour.

In the preoperative side view of the abdominoplasty,

Rectus diastasis and skin laxity
Caused a diagonally downward protrusion,

And adhesions above the cesarean scar
Created an uneven, irregular surface.

The overall heaviness of the abdomen
Was noticeable,
And the waistline lacked a natural flow.

These concerns were addressed through abdominoplasty.

As a result, a firm anterior abdomen
With reduced protrusion,

And a slim, lightweight waistline silhouette
Free from skin sagging,

Could be clearly observed.

During the recovery process after abdominoplasty,
Consistent care is essential

To allow the incision scars
To settle stably and smoothly.

This process was fully explained to the patient,
Emphasizing that proper aftercare

Plays a key role in maximizing surgical outcomes,

And follow-up visits were scheduled
According to the patient’s recovery timeline and condition.

With continued postoperative care
And stable weight maintenance,

An even more natural
And stable abdominal contour

Can be expected over time.
