Many patients who visit our clinic
to address lower body obesity through liposuction
often hope to achieve a straight, slim leg silhouette
with a visible thigh gap when standing naturally.

However, creating a thigh gap is not guaranteed
for all patients with lower body fat,
even with thigh liposuction alone.

This is especially true for patients with muscle-dominant thighs,
who often expect dramatic results
without considering muscle influence.
For such cases, thorough preoperative consultation is crucial.

During body analysis, we evaluate
muscle mass, fat distribution, and bone structure
to determine whether liposuction alone can achieve the desired outcome.
Only by planning a customized surgical design
based on individual body type and proportions
can we deliver a truly satisfying result.

Today’s featured case involves a patient
with muscle-dominant thighs.
She underwent liposuction on the inner thighs, outer thighs (saddlebags),
and Chanel line
,
combined with botox injections to the muscle-heavy areas,
which led to a smoother and slimmer leg contour.

Let’s take a closer look
at her before-and-after photos to see the transformation.

[Patient Information]
Age: Late 20s
Occupation: Office worker
Height: 154 cm
Weight: 55 kg
BMI: 20
Results may vary, even among patients with similar body types.

This patient had a normal BMI,
and appeared slim overall,
but had localized fat deposits in the thighs
that made her feel self-conscious.
She sought thigh liposuction
to refine her leg shape and silhouette.

Body type analysis for this patient revealed:

1.She had a muscle-dominant thigh structure,
with low fat volume but well-developed thigh muscles.

2.In particular, her inner thighs showed significant adductor (inner thigh) muscle development,
which meant that if liposuction was performed too aggressively,
the muscle contours could become overly prominent
and result in an uneven appearance.

3.Additionally, due to localized fat
in the inner knee area and the Chanel line,
her thigh shape showed visible curvature,
creating the visual illusion of shorter legs.

Based on these findings,
we proposed the following thigh liposuction plan:

1.Perform targeted liposuction on the inner thighs, outer thighs (saddlebags),
and the Chanel line,
to evenly reduce fat and sculpt a slimmer, more balanced leg shape.

2.For the inner thighs,
we planned to combine liposuction with adductor botox injections
to reduce muscle prominence
and create a smoother, more refined inner leg line.

Below are the before-and-after photos
taken 1 month after the thigh liposuction procedure.

[Surgical Areas & Fat Volume]
Procedure areas: Inner thighs, outer thighs (saddlebags),
Chanel line, and adductor botox
Total fat removed: 1,600cc

In this case, the patient underwent liposuction on the inner thighs,
saddlebags, and Chanel line to address the friction and rubbing
caused by thick inner thighs,
as well as to correct the lack of balance in her lower body.

By carefully removing excess fat from these key areas,
she aimed to achieve a slimmer, more streamlined leg line
with improved comfort and visual harmony.

As shown in the comparison between
medical photos and selfies taken after the thigh liposuction,
the patient’s muscle-dominant thighs were successfully transformed
into a more straight, thigh-gap-defined leg line.

In particular, liposuction targeting the inner thighs,
saddlebags, and Chanel line helped balance
the lower body proportion, while also correcting the curve
from the inner thighs to the knees,
creating a longer, straighter leg silhouette.

At around 1.5 months post-op,
some bruising was still visible.
This was mostly due to abdominal liposuction
performed three weeks earlier,
with bruising and swelling descending into the thighs
because of gravity.

Over time, the bruises improved,
and by three months, the patient could wear shorts
and exposed clothing with no visible marks.

If concerned about incision pigmentation,
we recommended laser toning as part of post-op care,
to promote a smooth and even recovery.
