Many women struggle with lower-body fat accumulation
and feel they have a “lower body–dominant” figure.

Despite efforts like leg workouts, stretching, and massage
to promote circulation and improve shape,
it’s not always easy to achieve the leg line they want.

Even if you’re not overweight,
differences in body structure and fat distribution
can lead to an unbalanced silhouette,
making certain areas appear heavier than they are.

That’s why many women turn to thigh liposuction,
which directly removes localized fat and helps reshape the legs
into a slimmer, more balanced appearance.

However, the thigh area has distinct muscle and bone structure,
and simply reducing volume without considering
muscle definition and proportions
can lead to uneven or overly muscular-looking results.

Even with similar body specifications,
results of liposuction surgery may vary.

The patient featured in today’s case
experienced a weight gain of nearly 8kg
within two months before surgery,
which worsened her lower-body-heavy silhouette.
She visited our clinic with the goal of achieving
slimmer thighs through thigh liposuction.

Based on the preoperative consultation,
she had good skin elasticity across the lower body
and a moderate amount of muscle,
indicating that the liposuction effect would be favorable.
However, because of fat fibrosis,
she was at a higher risk of swelling and firmness
after surgery.

In addition to the thighs,
the patient was also concerned about the hip line,
where accumulated fat emphasized the back of the thighs
in an unbalanced way.

To improve the entire lower body line
in harmony with the upper body,
we recommended hip line liposuction as well.

In this case, we explained that
a small amount of fat should be intentionally left
underneath the buttocks
to prevent sagging
or the formation of banana folds,
which can occur when too much is removed.1

After thigh liposuction and hip line liposuction,
we anticipated that the calves might look relatively bulky
compared to the now slimmer thighs and buttocks.

Rather than performing calf liposuction,
we opted for calf botox to reduce the size
of the developed calf muscles,
helping the legs appear as straight and slim as possible.

Below, we’ll review the photos taken
two months after the patient’s thigh liposuction,
hip line liposuction, and calf botox,
to assess the improvement in her lower-body-heavy body type.

Through thigh liposuction, hip line liposuction,
and calf botox, we reshaped areas like the
thighs and buttocks that are difficult to improve
with diet alone, adjusting them in better proportion.

Fat concentrated in the heavy buttocks
and along the outer thighs (saddlebags)
previously a major concern for the patient—
was effectively reduced,
and the calves were smoothed into a straight shape,
completing the overall transformation.

In the selfie photos taken and provided by the patient
after thigh liposuction, the degree of transformation
can be seen more clearly.

We removed fat from the inner thighs, outer thighs (saddlebags),
and buttocks, adjusting the volume
to match the patient’s body proportions.

Unlike general weight loss, which often fails to change
the actual shape of the legs,
this procedure allowed for a slim, elongated look
with a visible thigh gap.

However, as previously explained,
this patient had moderate skin elasticity
and muscle mass,
but also had fibrotic fat,
which was expected to cause prolonged swelling
compared to average cases.

We once again emphasized that consistent aftercare,
including light walking, massage,
and home care routines,
can help maximize the effects
of thigh liposuction surgery.
