The patient is 158 cm tall, weighs 54.5 kg, and has a bmi of 21.8. About 7 to 8 years ago, she had previously undergone thigh liposuction at another clinic.

Although she had the procedure, she felt that only the line was slightly refined, while her main concerns — her thighs and hips — remained unchanged.

She continued to exercise steadily, but over time, her weight gradually increased, causing fat to accumulate again in her thighs.

In search of a thigh gap, she decided to visit our clinic.

In liposuction surgery, it’s important to achieve a final result with a balanced and slim line.

Depending on the look the patient desires, the areas for maximum fat removal and areas to retain volume are carefully designed before surgery, while also considering the balance of the legs to create a more dramatic line.

Thigh liposuction results can vary depending on the patient’s fat volume, muscle mass, and leg shape.

That’s why it’s essential to closely assess the patient’s condition before surgery to ensure a safe procedure and well-settled, satisfying results.

The patient’s thighs showed very little muscle and appeared to be predominantly fat-based. It was expected that liposuction would deliver satisfying results.

Additionally, the patient had a hip dip body type, so to improve this, we recommended a surgical plan that included pelvic fat grafting to add volume along the thigh line connected to the pelvis for a natural contour.

Hip dips often appear in body types with well-developed muscles, where the upper pelvic area is sunken, making the saddlebag area look more prominent.

By adding volume to the area that had been a source of stress for the patient through pelvic fat grafting, and performing thigh liposuction at the same time, a more balanced and harmonious body line could be achieved.

However, the pinch test showed that the skin elasticity was moderate to low, with noticeably reduced elasticity.

Since the patient’s skin was also relatively thin, we explained in advance that there might be some loose skin remaining after thigh liposuction.

Because of the reduced skin elasticity, instead of suctioning near the hip line, we recommended liposuction along the inner panty line area.

A total of 2900cc of fat was removed from the inner thigh panty line area, followed by pelvic fat grafting of 180cc on each side.

As mentioned earlier, we clearly informed the patient in advance that due to low skin elasticity, some remaining loose skin could be expected after the procedure.

In the patient’s pre-surgery photos, it is evident that due to a past liposuction experience and weight gain, fat had accumulated roundly in the front of the thighs, while the back showed uneven and lumpy fat distribution, highlighting a lack of elasticity.

At our clinic, during the thigh liposuction revision surgery, we evenly removed the remaining fat across the entire thigh, resulting in a slimmer and straighter front thigh post-surgery.

For patients like this, where fat has accumulated around the thighs and hip line, fat grafting to the pelvic area can be used to add volume, which helps create a more balanced and natural contour. This combination of fat grafting and liposuction leads to a synergistic effect, making the legs appear slimmer and longer than with liposuction alone.
