When it comes to enhancing a woman’s body shape,
the pelvic line plays an essential role
in creating more balanced proportions.

As the weather warms up
and clothing gets lighter,
many people start doing various lower body workouts
to achieve a smoother, more toned figure.

However, building gluteal volume
requires long-term effort,
and because of the structural characteristics
of East Asian body types—
with narrower hip width
and generally less buttock volume
compared to Westerners—

achieving a perfectly balanced hip shape
can be extremely difficult.

That’s why, for those who feel self-conscious
about their flat or narrow hip lines,
a great solution is pelvic fat grafting:
removing excess fat through liposuction
from areas like the abdomen or thighs,
then injecting a portion of that fat into the hips
to restore natural volume and improve curves.

Today’s case features a patient
who underwent revision thigh liposuction
along with hip dip fat grafting,
resulting in a more feminine, well-balanced leg line.
Let’s review the case and examine the transformation.

[Patient Info]

Age: Early 30s

Occupation: Office worker

Height: 155 cm

Weight: 44 kg

BMI: 18

Even among patients with similar body profiles,
results from liposuction may vary.

She previously underwent thigh liposuction
at another clinic one year ago,
but was dissatisfied with the results,
which caused ongoing stress.

According to the body analysis,
this patient had previously undergone thigh liposuction
at another clinic, but a small amount of fat remained
in the inner thighs.

As for her buttocks, the fat volume was quite high,
and following the initial procedure,
the weight of her buttocks caused the formation
of a gluteal fold beneath the buttocks.

We therefore proposed the following
revision thigh liposuction plan.

The plan involved removing the residual fat
from the inner thighs to restore lower body balance.

To correct the imbalance caused by
the heavy buttocks, we performed hip line liposuction
to contour the upper and outer hip areas.

To create a more feminine leg shape,
we used the fat harvested from the inner thighs
and hip line to perform a hip dip fat graft
that enhances the pelvic curve.

However, we explained that the amount of fat
from the thighs and hip line alone
might not be sufficient for optimal graft volume
to the hip dips.

Therefore, we included love handle liposuction
in the plan to harvest additional fat
for the hip dip grafting.

Additionally, to minimize the adductor development
in the inner thighs and help create a straighter leg shape,
we recommended adductor Botox
following the revision thigh liposuction.

Below is a comparison photo
taken before and two weeks after surgery.

[Surgery Areas & Fat Volume (Liposuction + Grafting)]

Liposuction areas: Inner thighs, love handles, hip line
Total fat removed: 1500cc

Fat graft areas: Hip dips, gluteal fold
Hip dip graft volume – Right: 85cc, Left: 60cc
Gluteal fold graft volume – Right: 14cc, Left: 16cc

The patient aimed to restore lower body balance
through revision thigh liposuction,
while also creating a more feminine silhouette
with soft curves and contours in the legs.

As shown in the photos taken
two weeks after surgery,
the stubborn lower body fat
that had been bothering the patient
was successfully removed,
resulting in a slimmer, more proportionate leg line.

In addition, by transferring the removed fat
to the hip dip area, which lacked volume,
we were able to sculpt a fuller,
more feminine pelvic line.

Pelvic fat grafting—using the patient’s own fat—
is a great option for volumizing flat areas,
as it reduces the risk of complications or foreign body reactions,
while also allowing the volume to last longer
compared to synthetic alternatives.

Of course, results can vary individually,
but with thorough consultation
and a treatment plan developed by a skilled,
experienced medical team,
satisfaction with the outcome can be very high.
