Thigh liposuction is no longer focused
solely on reducing circumference.
It has evolved into a procedure aimed at creating
the most ideal lines by considering
overall upper–lower body balance.

In common cases where the thighs are thick
but the pelvic volume is insufficient,
the lower body may look unbalanced
and the overall silhouette lacks smoothness.
In these situations, combining
liposuction with fat transfer
becomes necessary.

Some may wonder why volume would be added
when the goal is to look slimmer.
However, areas with excess fat
need reduction to refine the shape,
while areas lacking volume—which can actually
make the body look bulkier and uneven—
need gentle filling to create
a stable and natural curve.

Below, we will explain the changes
observed in a patient who corrected
lower body imbalance through
thigh liposuction and
hip dip fat transfer,
improving both lower-body obesity
and hip dip indentation.

Even with similar body characteristics,
results from liposuction may vary.

This patient regularly practiced yoga
and pilates, but experienced sudden
weight gain over six months, leading to
thicker thighs and a more noticeable
hip dip indentation,
which brought her to our clinic.

She felt that exercise alone could not
slim the thighs to her desired shape,
nor could it create improvement
in the pelvic line.
For these reasons, she decided to undergo
thigh liposuction combined with
hip dip fat transfer.

During the consultation, the patient’s thighs
had a high overall fat volume,
causing the inner thighs to touch.
Because she also had substantial muscle mass,
this contributed to added thickness
and a heavier appearance.

Compared to the outer thighs,
the hip dip area was noticeably hollow,
creating an uneven and indented shape
along the outer thigh line
when viewed from the front.

With increased thigh fat volume
and insufficient hip dip fullness,
the imbalance in thigh thickness and contour
became more pronounced,
making the legs appear shorter.

Considering these body characteristics
as a whole, thigh liposuction
and hip dip fat transfer
were performed together.

In the pre-surgery front view,
the inner and outer thigh fat
made the horizontal width appear broad,
and the thighs were touching each other,
which was clearly visible.

The pelvic area lacked volume,
creating an inward hip dip line,
which made the outer thighs
look even more raised and prominent.

With thigh liposuction and
hip dip fat transfer,
the thigh circumference was noticeably
reduced after surgery.

As the inner thigh fat was refined,
a natural gap formed between the legs,
creating a much slimmer-looking line.

Additionally, with outer thigh liposuction
and natural volume added to the hip dip,
the pelvis and thighs now connect smoothly,
and the starting point of the legs
looks more stable — resulting in
an overall improvement in
lower-body proportions.

From the side view before surgery,
the forward and backward thigh bulging
made the lower body appear firm
and heavy in overall shape.

In the oblique view,
the hip dip indentation connected into
the thigh line, creating an uneven
and irregular pelvic contour.

However, with thigh liposuction and
hip dip fat transfer,
these issues were corrected, and not only
was the thigh circumference reduced,
but the previously noticeable heaviness
visually disappeared as well.

In the selfie photos the patient
took herself, the difference in how
everyday clothing fits is clearly visible.

The overall thigh volume has decreased
significantly, and the hip dip area
has been filled to create a balanced
and completed lower-body line.

In this patient’s case, the sudden
increase in weight was a concern,
but the fat concentrated in the lower body
made the upper–lower body proportions
look uneven. Combined with the imbalance
between the thighs and hip dips,
improving the line through exercise alone
was difficult.

With thigh liposuction and fat transfer
performed together, the patient achieved
not only a noticeable reduction
in thigh size,

but also an improvement in the entire
lower-body proportion and silhouette
through changes in the pelvic line,
resulting in a much more stable contour.
