Abdominoplasty for post-pregnancy abdominal laxity, Lesarts Plastic Surgery

After pregnancy, childbirth,
or significant weight fluctuations,
loss of abdominal skin elasticity
often leads to sagging
that is difficult to reverse naturally.

In many cases, this is accompanied
by rectus diastasis,
where the muscles supporting the abdominal wall
become stretched and weakened.
As a result, the abdominal contour
appears uneven,
with more pronounced sagging.

When excess abdominal fat
is added on top of these changes,
the entire abdominal line
inevitably looks heavier
and less defined.

These concerns are rarely resolved
through exercise or muscle training alone.
Because abdominal shape and firmness
are influenced by the skin layer, fat layer,
and muscle layer
together,
effective improvement requires
a comprehensive approach.

By correcting loose skin
and repairing stretched rectus muscles,
while refining the overall line,
abdominoplasty becomes
a fundamental solution
for restoring a smoother,
more stable abdominal contour.

Even with similar body specifications,
surgical results may vary by individual.

Despite consistent exercise and dietary management,
this patient experienced persistent
rectus diastasis and skin laxity
following two C-section deliveries.
As these concerns did not improve,
she decided to proceed with abdominoplasty.

The patient was particularly concerned about
the heaviness caused by sagging abdominal skin,
abdominal protrusion from rectus diastasis,
and the unstable line from the waist to the hip dip area.

As an international patient,
she hoped to proceed with treatment
on the shortest possible schedule,
requesting abdominoplasty,
abdominal liposuction,
and hip dip fat transfer
to be performed together.

Before surgery, the patient’s abdomen
showed a weakened central abdominal line,
with the midline appearing slightly widened
rather than firmly supported.

Although rectus diastasis was not severe,
reduced strength in the surrounding abdominal muscles
contributed to this appearance.

In the lower abdomen, adhesions from the C-section scar
and skin laxity appeared together,
causing visible skin overlap.
With additional abdominal fat present overall,
the waistline lacked smooth definition.

To address these abdominal concerns,
the following surgical plan was explained.

Surgical plan

1. Abdominoplasty (S-type, umbilical floating)

-Corrected skin sagging focused on the lower abdomen

-Removed the existing C-section scar

-As upper abdominal skin elasticity was preserved,
the umbilical floating technique was applied

-Improved abdominal sagging
while maintaining a natural umbilical position

2. Rectus muscle repair (partial repair)

-As rectus diastasis was mild,
the fascia was partially tightened
to restore abdominal wall support

-Improved the appearance
of a widened abdominal center

C-section scar removal and adhesiolysis

-Released adhesions around the C-section scar

-Removed the existing scar and
corrected surrounding skin folds
through skin lifting

4. Abdominal liposuction + hip dip fat transfer

-Removed fat from the anterior abdomen, flanks,
and love handles

-Performed hip dip fat transfer
to create a smooth curve
from the waist to the pelvis,
improving overall body balance

Below, we will review the before and after photos
taken approximately 3.5 months after surgery,
to explain the results achieved through
mini abdominoplasty, abdominal liposuction,
and hip dip fat transfer.Below are photos taken 8 months after abdominoplasty.
We will review the surgical results, including
C-section scar removal, improvement of uneven texture
from adhesions, lifting of severe sagging
from the upper abdomen, correction of lower abdominal overlap,
and rectus diastasis repair.

Mini abdominoplasty (umbilical floating)
Abdominal liposuction
Hip dip fat transfer

Before mini abdominoplasty,
the lower abdomen showed not only skin laxity
but also firm adhesions around the C-section scar.
This caused the lower abdomen to appear thick,
with visible downward heaviness.

Although upper abdominal skin elasticity
was relatively well preserved,
the abdomen appeared widely spread at the center,
making the waistline look dull and undefined.

Through mini abdominoplasty,
sagging of the lower abdomen was corrected,
and the C-section scar with adhesions
was removed together.

The new abdominoplasty scar,
shown at 3 months post-surgery,
is expected to become progressively
lighter and thinner over time.

With abdominal liposuction,
the waistline became clearly indented,
and the body’s center of balance
shifted upward rather than downward,
creating a more stable silhouette.

Through hip dip fat transfer,
depressed pelvic areas were filled,
allowing a smooth curve from the waist to the pelvis
and resulting in a more proportionally
balanced and harmonious body line.

When comparing the before and after bending posture,
both skin sagging and waistline concerns
show clear and effective improvement.

Lower abdominal skin sagging and overlap
that had formed above the C-section scar
were removed through mini abdominoplasty,
resulting in a firm and lifted appearance.

As the overall waist circumference decreased,
the effect of restored skin elasticity
became even more pronounced.

Through surgery, a natural and youthful upper body silhouette
was successfully achieved.

From the side view,
the transformation from protrusion and sagging
to a smooth and firm contour
is clearly visible.

Skin that had overlapped
above the underwear waistband
was fully corrected through
mini abdominoplasty
and abdominal liposuction.
The lower abdominal line
now shows restored firmness
and elasticity.

Over approximately 3.5 months of follow-up,
recovery progressed without major issues.
The desired correction of sagging
was successfully achieved,
and the abdominal and waist lines
settled into a stable, feminine shape.

Through abdominoplasty,
excess sagging skin was removed.
With rectus muscle repair,
the widened abdominal shape
was gathered forward
and firmly lifted.

As a result, the abdominal center
is now well supported,
showing a natural, balanced body contour
with improved stability.
