This patient is a university student majoring in physical education.

Despite participating in school sports and receiving personal training sessions, she was stressed by the lack of improvement in her thighs.

After considering various options, she decided that liposuction would be the most effective solution.

This patient is 162 cm tall, weighs 53.4 kg, and has a BMI of 20.3. She had good skin elasticity and a high muscle mass, as she is a university student majoring in physical education.

Although her muscle ratio was high due to regular training, she also had a significant amount of fat, particularly concentrated in the “saddlebag” area of the thighs. This made her a good candidate for thigh liposuction, with the potential for dramatic improvement.

Fat accumulation was most prominent in the upper thighs, so we recommended including the area beneath the buttocks in the procedure.
This is because when fat is removed from the thighs, which normally provide structural support to the buttocks, the weight of the buttocks can shift downward. This can deepen or lengthen the natural fold beneath the buttocks, potentially creating the appearance of sagging.

To prevent this and ensure a more lifted look, we recommended including the lower buttock area in the liposuction.
We also analyzed the pelvis, calves, and muscle structure in order to design the procedure precisely. In this patient’s case, her leg bones were naturally straight, which led us to expect a high level of satisfaction with the surgical outcome.

As a result, thigh liposuction was performed with an estimated fat removal volume of 4,000cc.

Before undergoing thigh liposuction to achieve straighter legs, noticeable fat accumulation was observed in the saddlebag area and upper inner thighs.

In the photos taken 15 days after surgery, swelling and bruising had descended down to the calves—an area that was not operated on—due to gravity. However, a visible reduction in fat volume in the saddlebag and inner thigh areas could already be seen.

At around 4 months post-surgery, the noticeable swelling and bruising had significantly improved, and her legs had come much closer to the desired contour.

Additionally, refining the saddlebag line gave the visual effect of longer, slimmer legs.

She mentioned that even on the day of the surgery, she could already see a noticeable reduction in thigh volume when checking her silhouette at home.
Seeing the immediate change, she felt it was almost a waste of time having spent so long doing personal training just to improve her thighs.

Before starting a diet, it’s helpful to determine whether you have a fat-dominant or muscle-dominant body type.
Here’s a simple self-check method you can try:

  1. While standing in a relaxed posture (arms at your sides), check the thickness of the fat on your upper arms or thighs.
  2. Then, tense your muscles or hold a fixed posture, and check the thickness again.

If there’s little difference in thickness between relaxed and flexed states, you may have a fat-dominant body type.
If there’s a significant difference, it could indicate a more muscular body type.
