For many men, confidence
is built from surprisingly simple elements.
A well-fitted shirt, trained shoulders,
and a firm chest with subtle muscle definition
visible even under a T-shirt.

However, when gynecomastia is present,
the issue goes beyond a lack of confidence.
It can become a deeper concern,
especially for teenagers, men in their early 20s
preparing for military service,
or those just starting their careers,
often leading to psychological stress and pressure.

Rather than withdrawing,
seeking early diagnosis and consultation
can be a positive and empowering step.
While the final decision for surgery
always belongs to the patient,
knowing that a solution exists
can itself provide meaningful reassurance.

Furthermore, by improving gynecomastia

This patient had struggled with these concerns
for a long time and was diagnosed
with true gynecomastia through examination.
Through gynecomastia surgery,
he was able to achieve

The patient had experienced chest enlargement
and nipple protrusion since his teenage years,
causing long-term discomfort and concern.
Despite consistent exercise and weight control
after reaching adulthood,
there was little change in chest shape.

Visually, the nipple–areola complex
appeared widened and prominent.
On palpation, a firm mass was felt
beneath the nipple area.
Although there was no pain,
this suggested true gynecomastia,
rather than simple fat accumulation.

During consultation, ultrasound examination
confirmed the presence of glandular tissue,
showing signs of glandular overgrowth.
Even with adequate muscle mass,
the protruding chest made the upper body
appear heavy and unbalanced.

The patient reported significant stress,
especially when wearing fitted shirts.
Based on these findings,
liposuction alone was unlikely
to provide sufficient correction.

A combined approach of gland excision
and liposuction for gynecomastia
was therefore recommended.
This plan aimed to reduce chest size
while creating a firm, athletic chest contour,
often referred to as male chest contouring.

1. Gland excision surgery
Through a periareolar incision,
the developed glandular tissue was removed
to reduce overall chest volume.
The goal was to create a flatter,
more balanced chest contour.

At the same time, the overly prominent
nipple–areola complex was refined
to achieve a smoother
and more natural chest line.

2. Combined liposuction
Fat distributed along the lower chest
and lateral chest areas was removed
to supplement the results of gland excision.

Rather than correcting gland-related protrusion alone,
this approach was designed to create
a smooth transition from the chest wall
to the axillary line

resulting in a more natural side profile.

3. Adhesion prevention and chest contour refinement
After gland removal, measures were taken
to prevent irregular adhesions.
Additional tissue repositioning was performed
to correct depressions and flat areas,

with the goal of achieving
a balanced and athletic male chest silhouette
that looks natural even under a T-shirt.

Using photos taken 2 months after gynecomastia surgery,
we can clearly compare the before and after results.
Before surgery, an enlarged chest with
protruding nipples and areola
created an awkward and uncomfortable chest contour.

After gynecomastia surgery,
these concerns were effectively corrected,
resulting in a much cleaner and more refined chest line.

As the chest volume was reduced,
there were no visible depressions or irregularities.
The upper chest appears firm,
while the central, lower, and lateral chest areas
now transition naturally
into the abdomen and flank lines.

The T-shirt fit was one of
the patient’s primary concerns.
Correcting chest protrusion
and the bulging nipple–areola complex
into a natural, masculine shape
was a key goal of the surgery.

As the patient exercised consistently,
there were also certain expectations
regarding the final chest appearance.
At 2 months after gynecomastia surgery,
the T-shirt fit shows a smoothly refined chest,
with a firm upper chest line
that resembles the result of regular training.

With no remaining protrusion,
the chest contour appears stable and balanced.
This allows confidence in wearing
any type of clothing.

Having struggled with gynecomastia
since his teenage years,
this case focused not only on size reduction
but also on achieving a satisfying line refinement.
