Gynecomastia is a condition where the male chest
becomes unnecessarily enlarged, causing significant
discomfort in daily life as well as a strong
psychological burden.

In situations where chest workouts or
weight loss do not improve the condition,
it may not be caused simply by body fat,
which is why an accurate diagnosis
and proper treatment are required.

The patient featured in today’s
gynecomastia surgery case had noticed
an increase in chest size since his early 20s
and suffered for a long time to the extent
that even wearing a simple T-shirt
felt burdensome and stressful.

Here, we will explain how his symptoms
were improved through surgery.

Although the patient normally does not experience
significant weight changes, the sudden increase
in chest size and the noticeable prominence
of the areola around the nipple caused major
limitations in daily life and clothing choices,
becoming a significant source of stress.

To resolve this discomfort fundamentally,
the patient visited Lesarts Plastic Surgery
for proper evaluation and treatment.

With visual inspection alone,
both sides of the chest were noticeably enlarged,
showing a clear protruding shape.

In particular, the area around the nipple and areola
appeared swollen and raised,
and upon palpation, firm glandular tissue
could be felt in the region.

An ultrasound examination was performed
for accurate assessment, and glandular overgrowth
was confirmed on both sides of the chest.

1. Gynecomastia glandular excision

Through an areolar incision,
the overgrown glandular tissue was removed
to reduce chest size and create
a more balanced male chest contour.

2. Chest liposuction

Liposuction was performed on the
lower and lateral chest areas
to supplement the chest reduction
beyond glandular excision alone
and to maintain a smooth, natural
male chest contour.

3. Chest tissue rearrangement (pectoral contouring)

To prevent a hollow or sunken appearance
after gland removal, tissue rearrangement
(pectoral contouring) was planned
to achieve a more masculine chest shape.

4. Monitoring of hair-loss medication

The patient had been taking finasteride,
a hormone-influencing medication
for hair-loss treatment, for a long period.

Because such medication can have
a relationship with gynecomastia,
the surgical decision was made
after reviewing examination results
and clinical findings together.

The patient was also informed that
continued medication use could still affect
chest tissue after surgery and that
regular postoperative monitoring
would be necessary.

At approximately 2 months after gynecomastia surgery,
the patient’s photos were taken to confirm
how the chest protrusion had improved
and to evaluate the surgical results
through a clear before-and-after comparison.

Before surgery, the enlarged chest
and protruding nipple area were noticeable,
and from the side view, the protruding line
appeared even more prominent.

Through surgery, the overgrown
glandular tissue was removed,
along with excess fat from the lower
and lateral chest, reducing the excessive
chest volume and refining the
male chest contour.

With the improvement of nipple protrusion
as well, the chest was corrected into a smooth
and flat shape, resulting in an overall
more masculine appearance.

In the appearance with a T-shirt on,
the previously embarrassing chest protrusion
has disappeared, and the overall fit of the clothing
looks much smoother than before.

Before gynecomastia surgery,
the patient felt very uncomfortable
wearing even a single T-shirt.

Especially from the side view,
not only the enlarged chest but also the
forward-projecting nipple made it difficult
to wear a simple T-shirt comfortably.

However, at 2 months after gynecomastia surgery,
all these issues and discomforts have been resolved,
and a natural and clean T-shirt fit
can now be observed.

This patient had experienced significant
discomfort and psychological stress
for a long time due to gynecomastia,
particularly when choosing clothing.

Through gynecomastia surgery,
the overgrown glandular tissue and fat
were removed, reducing chest size,
improving nipple protrusion,
and eliminating the firm nodules.

Now, the patient can enjoy a natural
and tidy upper-body silhouette
even when wearing a T-shirt,
and expressed a high level of satisfaction.

Gynecomastia should not be considered
a problem caused only by weight gain
or lifestyle habits; it requires monitoring
of changes in glandular tissue influenced
by hormonal factors.
