Stories of men struggling with gynecomastia,
who see no change in their chest shape or size
despite exercise and diet control,
often appear in various fitness communities.

Even men who are not obese
may experience a large chest
and swollen area around the nipple,
leading to concerns that cannot be solved
by chest workouts alone.

However, many hesitate about
gynecomastia surgery
because of concerns about the timing
or season of the surgery.

First and foremost,
an accurate diagnosis is essential,
and it’s important to know that
the season of surgery
does not significantly affect the results.

Today, through a patient case
who struggled with chest protrusion
due to gynecomastia,
we will introduce the diagnosis of true gynecomastia,
the surgical improvement process,
and the changes that followed.

The patient is a male office worker in his early 30s,
who has experienced gynecomastia symptoms
since his teenage years.

Although he maintained a consistent weight through exercise,
the size of his chest and the lump around the nipple
showed no improvement,
leading him to seek a proper diagnosis.

What bothered him most was the pronounced chest line
when wearing a T-shirt,
and the protruding shape of the nipple.

He also avoided activities involving upper body exposure,
which lowered his confidence
in social settings.

Before surgery, the patient’s condition was examined,

showing clear hyperplasia of the glandular tissue
centered around both nipples.
Especially from the side view,
a prominent chest contour
protruding below the nipple was observed.

Fat accumulation was also present
in the lower and side chest areas,
which further emphasized the chest protrusion.

Based on this, the patient was diagnosed
with true gynecomastia
corresponding to Simon Grade 2A.

The necessary surgical methods to improve the symptoms included:

1.Glandular tissue excision
Removing the glandular tissue
directly affecting chest size,
reducing the protrusion,
and creating a flatter overall chest contour.

2.Liposuction
Since fat was also accumulated
in the lower and side chest areas,
liposuction was performed simultaneously
to smooth the chest contour
and adjust the volume.

3.Adhesion prevention and depression avoidance
After removing glandular tissue,
preventing dimpling or adhesion
around the nipple area caused by the empty space.

Below, we will review the chest line changes
and recovery status
observed 2 months after gynecomastia surgery.

Comparing the appearance
2 months before and after gynecomastia surgery,

the swollen chest contour around the nipple
has been refined,
making the overall upper body line
much flatter and more defined.

Especially from the side view,
the protruding chest contour
that extended to the lower chest before surgery
is now smoothed out,
creating a more masculine chest line.

Most importantly,
the side chest line was also refined,
resulting in a toned thoracic silhouette,
as if shaped by exercise.

Before surgery, the patient’s nipple
protruded noticeably under T-shirts,
but after surgery,
the nipple height was stabilized
without any protrusion.

Even wearing just a white T-shirt,
the patient can confidently
stand with relaxed shoulders,
showing a natural chest fit.

In true gynecomastia,
the glandular tissue is overdeveloped,
which is unrelated to fat.
Therefore,
simple weight loss or exercise
rarely improves the condition.

In this case,
the glandular tissue was strongly developed
around both nipples,
with fat accumulation in the lower and side chest,
making the upper body contour appear bulky.

Considering these factors,
the combination of glandular tissue excision
and liposuction was performed.

As a result,
the central chest protrusion was completely refined,
and the upper body silhouette
extending to the side chest
became much smoother.

The overly swollen nipples were also
corrected to a natural, stable shape,
allowing a neat and confident T-shirt fit
without discomfort, even in thin clothing.

However,
for cases like this patient’s,
who experienced symptoms since their teens
and had long-lasting true gynecomastia,
both immediate surgical results and the time
for tissues to stabilize and skin to settle
are important.

So far,
the nipple position and chest contour
show very good recovery,
but to prevent adhesion or shape imbalance,
postoperative care is essential, including:

– Wearing a compression garment

– Maintaining stable weight

– Managing upper body activities

If these care steps are consistently followed,
the patient can expect a more natural
and stable upper body line
over time.
