Gynecomastia in Korea refers to a condition
where a man’s chest looks visibly
full and protruded like breasts.

Gynecomastia is diagnosed in two types
depending on the underlying cause.

First is pseudogynecomastia, caused by
fat accumulation in the chest area.
Second is true gynecomastia, caused by
abnormal growth of glandular tissue
due to hormonal influence.

If you try chest workouts or dieting
to improve gynecomastia symptoms,
but see no change in chest size,
it may be necessary to suspect
true gynecomastia.

In cases of true gynecomastia,
a firm lump can be felt in the chest,
the nipple and areola may look puffy,
and in some cases a tingling pain
can also be accompanied.

It often develops during adolescence,
when secondary sexual characteristics begin.
Symptoms may improve naturally during this time,
but if they do not, years of accumulated stress
can significantly affect quality of life.

Looking at the age range of patients
who undergo gynecomastia surgery,
it is becoming increasingly broad,
from teenagers to men in their 50s.

If you suspect gynecomastia symptoms
and feel discomfort in daily life,
active treatment can be helpful
both physically and psychologically.

This patient we will discuss today
also had gynecomastia symptoms
that began during adolescence
and continued into adulthood.

Despite consistent self-management,
there was no noticeable improvement,
so he ultimately decided
to proceed with gynecomastia surgery.

He shared that daily-life discomfort
was stressful, but more importantly,
he felt major limitations in clothing
and activities that exposed the upper body.
To reduce this psychological burden,
he chose surgical treatment.

During consultation, the patient’s chest shape
was clearly not flat, and showed
a visibly full, protruded contour.

The nipple and areola area appeared puffy,
and on palpation we also confirmed
a firm suspected glandular tissue mass.

To evaluate gland distribution
around the areola and across the chest,
we performed an ultrasound examination,
confirmed overgrown glandular tissue
and its overall range, and diagnosed
true gynecomastia, Simon grade 2A.

1. Gynecomastia surgery – gland excision

Through a periareolar incision,
we removed the overgrown glandular tissue
to reduce chest size and correct
the protruding nipple.

2. Gynecomastia surgery – chest liposuction

For volume in the lower and lateral chest,
we performed liposuction to support
a more natural chest reduction effect.

3. Male chest contouring –
tissue rearrangement and adhesion prevention

To prevent imbalance or depression
that can occur after gland removal,
and to create a firmer upper chest contour,
we combined tissue rearrangement
with an adhesion-prevention procedure.

4. Precaution – blood flow monitoring

In this patient’s case, blood flow
was confirmed to be relatively high,
so we carefully monitored bleeding status
during surgery and throughout recovery.

This patient had true gynecomastia,
classified as Simon grade 2A, and due to
overgrown glandular tissue along with fat
in the lower and lateral chest,
his chest looked enlarged and
nipple projection was also distinct.

To treat gynecomastia symptoms,
we combined gland removal, liposuction,
and tissue rearrangement to reduce
overall chest size and correct the contour
into a firmer, more defined chest shape,
resulting in a neat yet natural appearance.

As the glandular tissue was removed,
the previously puffy nipple and areola area
also settled down, and the embarrassing
chest protrusion that caused discomfort
completely disappeared.

When wearing a T-shirt, the patient’s
chest curves and the puffy shape
around the nipple and areola
were clearly visible, so he could not help
but feel significant discomfort
in everyday life.

In the photos taken two months after
gynecomastia surgery, the upper chest
looked firmer, as if he had worked out,
and the areas that once stood out in volume
naturally reduced, allowing him to regain
confidence in his T-shirt fit.

Although the patient had a note
of relatively high blood flow,
careful feedback checks and follow-up care
were provided from immediately after surgery,
and he is showing a stable recovery
without any specific issues.

Even at two months, when mild swelling
can still remain, the results are holding
in very good condition, and the patient
shared that he feels highly satisfied
because he has finally been relieved
from the long-standing psychological burden.
