For male adolescents who experience
noticeable physical changes different
from their peers, there is often
heightened sensitivity about body shape.

In particular, when there is chest protrusion
in the upper body, it can lead to
extreme self-consciousness and even
psychological withdrawal.

This is because such changes affect not only
how others perceive them but also play
a major role in shaping self-confidence.

The condition is called gynecomastia,
meaning breast enlargement in men.
If caused by hormonal imbalance,
it should be recognized as a medical
condition requiring treatment.
Clear awareness of this fact is crucial.

The patient we present today
is a man in his mid-20s
who had experienced childhood obesity
and from an early stage suffered discomfort
from the size and protrusion of the chest.

Although weight loss through dieting
reduced overall body weight,
the chest volume and
nipple-area protrusion
remained unchanged.

Like many patients with gynecomastia,
the greatest stress came during daily life—
especially when wearing thin clothing
or cotton T-shirts, where the chest
appeared rounded and projected outward,
resembling a female breast.

In addition, he reported a firm lump
palpable beneath the areola.

These findings suggested that it was not
a simple fat-related body shape issue,
but rather a condition requiring
accurate diagnosis
and surgical correction,
which led him to visit our clinic.

During medical consultation,
the patient’s chest showed a distinct bulge
centered around the nipples,
with volume extending downward,
making the chest contour look emphasized.

The areola was wide and thick,
and a firm lump was detected
beneath the nipple.

Ultrasound confirmed glandular overgrowth,
leading to a diagnosis of
true gynecomastia, Simon Grade 2B.

Based on this, we explained the following
surgical treatment plan:

1.Gland Excision
Through an areolar incision,
the enlarged gland tissue causing
chest protrusion would be removed,
reducing chest volume and correcting
nipple projection.

2.Liposuction
Fat would be removed from the lower chest
and lateral chest, ensuring the contour
flows smoothly and naturally
beyond just gland removal.

3.Adhesion & Inversion Prevention
To prevent possible nipple depression
or skin adhesion after gland removal,
deep tissue repositioning and
anti-adhesion measures were applied.

At 3 months post-op,
we reviewed recovery progress and
compared how the chest contour improved
through before-and-after photos.

When comparing the before & after results
of gynecomastia surgery, the protruding
chest contour centered around the nipples
was visibly corrected, giving the upper body
a neater and smoother silhouette.

Before surgery, the chest shape was
bulging downward, but after correction,
it became flat and improved into a
masculine ribcage contour.

Because not only the gland tissue
but also fat in the lower chest
and lateral chest was removed,
the overall bodyline was refined.

With tissue repositioning, the result also
showed a defined “muscle-like effect,”
creating a firm and athletic-looking chest.

When comparing the patient’s chest
in a T-shirt before & after surgery,
the nipple area that once protruded
was smoothed, and the shirt fit
no longer appeared awkward.

Gynecomastia surgery is not only about
reducing visible chest bulges but also
creating a balanced, masculine contour
for the upper body.

In this case, gland tissue under the nipples
was overdeveloped, while fat around
the lower chest and lateral chest
made the chest look large and saggy.

Thus, gland excision with liposuction
was performed to reduce chest size
and refine overall contour, ensuring
a natural, proportional result.

Because removing only gland tissue
can leave a flat or hollow chest,
a comprehensive surgical approach
was chosen, leading to a highly
satisfactory outcome.
