Gynecomastia, where a man’s chest protrudes
like a woman’s, is more than
just a cosmetic concern—
it causes stress and social anxiety
that interfere with everyday life.

Each patient differs in chest size,
glandular volume, nipple projection,
and skin elasticity,
so a precise diagnosis and
tailored surgical plan is key.

The goal is to treat the root cause
while restoring a natural, balanced chest line
with a smooth and masculine shape.

In this case, the patient struggled
with long-term social discomfort
and aesthetic concerns from gynecomastia.

Let’s review the before-and-after results
and analyze how chest shape improved
after the gynecomastia procedure.

This patient is an office worker in his early 30s,
who had been experiencing gynecomastia symptoms
since his teenage years.

Even throughout his 20s,
the symptoms continued.
During military service,
he lost about 20kg,
but saw no change
in the size or shape of his chest.

More recently, as his weight increased,
the chest protrusion became more noticeable,
leading to greater discomfort
in everyday life.

Not only tight-fitting clothing,
but even loose tops
emphasized his chest silhouette,
causing ongoing stress over his appearance.

This made dressing for work
especially stressful,
and even casual outings
became mentally burdensome.

Eventually, he concluded that
exercise and dieting alone
could not resolve the root of gynecomastia,
and decided to consult
Lesarts Plastic Surgery for a solution.

The patient’s chest protrusion
was visibly noticeable to the naked eye,
and the overall chest size and shape
appeared prominent and rounded.

To ensure an accurate diagnosis,
we performed an ultrasound examination,
which confirmed glandular hypertrophy
in both breasts.
He was diagnosed with true gynecomastia,
classified as Simon Grade 2A.

Based on the clinical findings,
Lesarts Plastic Surgery created
the following surgical plan:

1.Perform a gland excision
to remove the root cause—
overgrown glandular tissue—with precision.

2.Use liposuction to remove excess fat
from the lower and lateral chest,
which contributes to chest size and bulging contour.

3.Apply anti-adhesion treatment
and tissue repositioning
to prevent postoperative tissue adhesion
and avoid nipple depression.

Let’s now review the actual results
by comparing the before and
2-month post-op photos
to observe how the chest contour has changed.

Before surgery, the photos clearly show
enlarged glandular tissue in both breasts,
resulting in a bulging chest shape.
From the side view,
the lower chest protrusion
was especially noticeable.

However, 2 months after gynecomastia surgery,
with both gland excision
and lateral chest liposuction,
the chest volume was significantly reduced,
resulting in a flat and smooth,
masculine chest contour.

Even from the side,
the once-protruding chest line
has become firmer and more natural,
showing clear improvement
in the overall chest silhouette.

Even when wearing a white t-shirt,
the change before and after surgery
was clearly visible.

Before gynecomastia surgery,
he felt discomfort due to
visible chest volume through clothing,
but 2 months after surgery,
this concern was completely resolved—
now even a thin t-shirt fits
cleanly and naturally.

The lateral chest fat
that previously widened the chest contour
was also removed,
resulting in a slimmer and more balanced shape overall.

As much as the physical transformation,
his overall image became more confident.

However, since he had been dealing with
gynecomastia for many years,
ongoing post-op monitoring was still necessary.

We made sure the patient understood this,
and guided him to visit regularly
for checkups to help maintain long-term results.
