Gynecomastia surgery in Korea may sound unfamiliar,
but many men experience discomfort
from gynecomastia in daily life,
and a large number undergo
male breast reduction surgery for treatment.

When first hearing about gynecomastia,
or noticing symptoms personally,
most people tend to assume,
“Maybe I just gained weight?”
and think no further.

In many cases, however,
the enlarged chest is actually caused
by overgrowth of glandular tissue,
not simple fat accumulation.

This is true gynecomastia,
where a firm lump is felt,
the areola becomes puffy or raised,
and some patients even feel
a tingling or aching sensation.

Because this differs from
fat-based pseudogynecomastia,
weight loss or chest exercises alone
often cannot resolve
true gynecomastia symptoms.

For men, enlarged breasts often create
significant psychological stress,
leading to low confidence and pressure
from cosmetic concerns.
Treatment requires removing
the overgrown glandular tissue.

Below is a real gynecomastia surgery case,
with an explanation of how surgery
helps improve these symptoms
and restores confidence.

This patient reported that
since his early twenties,
his chest gradually enlarged
and the projection became
increasingly noticeable.

As the nipple and areola
also appeared swollen,
he faced limitations when choosing clothes,
and his stress continued to grow.

At first, he believed
consistent chest workouts
could reduce chest size
and reshape the appearance
without needing surgery.

Over time, however,
a firm lump developed under the areola,
and he realized lifestyle changes alone
could not correct his condition.
Seeking a fundamental solution,
he visited Lesarts Plastic Surgery.

Before gynecomastia surgery, this patient’s chest
showed clearly enlarged size on both sides
with a noticeably protruding chest contour.

In particular, both nipple projection
and a widened, puffy areola shape
were observed at the same time.

Because of the expansion caused
by overgrown glandular breast tissue,
true gynecomastia was strongly suspected.

On physical examination, a firm
and clearly palpable glandular lump
was confirmed beneath the areola.

For accurate diagnosis, we performed
an ultrasound examination, and the area
of concern was confirmed as
enlarged glandular tissue throughout.
Ultimately, this case was diagnosed
as true gynecomastia, classified as
Simon grade 2A.

1. Glandular tissue excision

Through a periareolar incision,
we removed the overgrown glandular tissue
to reduce overall chest volume
and correct the protruding nipple
at the same time.

2. Gynecomastia liposuction

We performed liposuction on the lower chest
and lateral chest area, targeting the fat
distributed around the breast.
This was planned to enhance the
chest reduction effect, which might be
insufficient with gland excision alone.

The goal was to create a smooth transition
from the chest to the abdomen and flanks,
resulting in a more natural body line.

3. Upper chest contour effect,
adhesion prevention, and tissue rearrangement

After glandular excision, we planned
an adhesion-prevention procedure
and tissue rearrangement around
the nipple–areolar complex to prevent
irregular adhesions or depressions,
while also creating an upper chest contour
similar to the effect of chest workouts
(“armor chest” effect).

4. Precautions related to medication history

The patient had been taking medication
for hyperlipidemia on a regular basis.

Because such medication can affect
hormones and overall metabolism,
we temporarily stopped the drug
before surgery and proceeded carefully,
taking his internal medical background
fully into consideration.

Below, we will review the postoperative photos
after gynecomastia male breast reduction surgery
to assess improvement of gynecomastia symptoms
and changes in the upper chest line.

Before male breast reduction surgery, this patient
had firm glandular tissue palpable around
both nipples, which caused the entire chest
to look enlarged and visibly
protruded forward.

In particular, fat volume in the
lower chest area created a shape
that seemed to sag along the ribcage,
so when wearing a T-shirt, the uneven
and nonflat chest line became
even more noticeable.

Two months after gynecomastia surgery,
the enlarged glandular tissue, which was
the main cause of the large chest,
was removed, leading not only
to a reduction in chest size
but also a significant improvement
in the protruding contour.

Combined liposuction of the lower chest
and lateral chest area helped the line
settle into a flatter, neater shape,
providing a synergistic effect
with the gland removal.

The sharp and pointy projection
around the areola and nipple
was also corrected, creating
a more natural chest contour.

Through tissue rearrangement
in the upper chest region,
the chest line transformed into
a more stable and defined
masculine upper chest appearance.

Before gynecomastia surgery, his T-shirt fit showed
a clearly protruding chest line and nipples,
making the overall appearance quite noticeable
and understandably embarrassing in daily life.

Two months after male breast reduction surgery,
these concerns were completely corrected,
and even with just a simple white T-shirt,
he could see a firm yet natural
and well-balanced chest silhouette.

This patient carried not only physical discomfort
but also a psychological burden
from his twenties into his thirties,
due to the appearance of his chest.

Because this was true gynecomastia,
which is difficult to resolve naturally
with chest workouts or diet control alone,
a surgical approach was necessary
to achieve meaningful improvement.

Through gynecomastia surgery,
the enlarged chest size and projection
at the center of his upper body
were effectively treated, restoring
overall stability and balance to his torso.
