It seems that recently,
more patients are planning

Gynecomastia surgery

before entering university,
before military enlistment,
or before returning to society
after discharge.

Before an important milestone,
it is natural to want to resolve
a long-standing complex.

Among men struggling with
a noticeably enlarged chest
that does not improve through
diet or chest workouts,

especially those in their early 20s
preparing for military service,
the decision to pursue surgery
is becoming more common.

Some patients visit alone.
Others come with their parents
for consultation.

When glandular tissue overgrowth
is the underlying cause,

exercise or weight loss alone
cannot reduce breast size.

In such cases,
surgical treatment
is the only definitive solution
for gynecomastia.

As introduced in previous cases,
we will explain today’s case
in detail through
a similar example.

This patient also attempted
weight loss

in hopes of reducing
his chest size.

However, the protruding
chest contour
and prominent
nipple–areolar complex

did not improve,
so he visited for
an accurate diagnosis
and proper treatment.

On ultrasound examination
for gynecomastia diagnosis,

both sides of the chest showed
overdeveloped
glandular tissue,

which was identified
as the primary cause
of enlargement.

Surgical treatment
was therefore necessary.

However, removing
only the glandular tissue

could have resulted in
an imbalanced shape

due to residual fat
in the lateral chest area.

For this reason,
lateral chest liposuction
was planned together

to reduce central protrusion
and restore natural
upper body balance.

Because the patient
had a larger build
and a thicker fat layer,

it was especially important
not only to excise
the glandular tissue,

but also to carefully refine
the surrounding fat layer

to create a well-shaped,
balanced chest contour.

1. Glandular tissue excision
(Periareolar incision)

Overgrown glandular tissue
was removed to reduce chest size

and correct protrusion
of the chest and nipple–areolar area.

2. Lower & lateral chest liposuction

Removing gland alone
can leave contour irregularities.

So chest liposuction
was performed together

to create a smooth,
balanced chest line.

3. Tissue repositioning
(Male chest contouring)

Because the patient
had a larger frame,

careful tissue rearrangement
was done

to prevent indentation
and create a firm,
athletic chest shape.

2 months after surgery,

showing the improvement

The patient had a solid,
well-built physique,

but the chest area
was disproportionately enlarged
and protruded,

preventing his natural body strengths
from standing out.

Through gynecomastia surgery,
the overdeveloped glandular tissue
—the main cause of enlargement—
was removed.

Additional liposuction
further reduced excess chest volume,
restoring better upper body balance.

With careful tissue repositioning,
no depressions were left behind,

and the chest transformed
into a firmer,
more masculine contour—
similar to a well-trained physique.

After gland removal,
not only was overall chest size reduced,

but nipple and areolar protrusion
was also corrected,

eliminating the previously
awkward and prominent chest appearance.

In everyday clothing,
the change becomes
even more noticeable.

As his natural strengths
begin to stand out,
the upper body now appears
balanced, firm,
and proportionate.

One of the most satisfying comments
we often hear from patients is,

“I can finally wear
a single T-shirt
without feeling self-conscious.”

Before surgery,
the central chest protrusion
was clearly visible
through clothing,

making the upper body
look unnatural.

After gynecomastia surgery,
the T-shirt fit
became completely natural,
with a smooth chest contour.

Many patients first try
chest workouts
or weight loss.

However, when the cause is
glandular tissue,
these efforts often fail—
or even make the chest
appear more prominent
as surrounding fat decreases.
