At first, noticing these symptoms can be surprising,
and there’s often a vague hope that
losing weight might solve the problem.

However, even after time passes and weight is lost,
the chest size may not decrease.
Instead, men find themselves exposed
to more stressful situations and discomfort.

In most cases, this is caused by the
overgrowth of glandular tissue, which requires
a precise medical diagnosis.

Glandular tissue enlargement is often due to
hormonal imbalance, which can be influenced
by puberty, menopause, certain medications,
or underlying medical conditions.

This means it can only be improved
through surgical treatment.
If daily life — at school, in the military,
or at work — is disrupted by discomfort
when wearing clothes, exercising, or
engaging in leisure activities,

In today’s gynecomastia surgery case analysis,
we will look at a patient in his late 30s,
whose chest enlargement and protrusion
worsened in his 20s due to
progressive male breast enlargement.

He recalled experiencing slight chest growth
during his teenage years and puberty,
but said it was not bothersome at the time.

In recent years, however, while taking
hair loss medication for treatment,
he noticed increased discomfort
in his chest area, making his gynecomastia
symptoms more noticeable.

He also experienced weight gain recently,
which led to more body fat accumulation
and further increase in chest size.

As a result, the areola protrusion
became even more prominent,
and he began to feel palpable lumps,
which ultimately led him to consider surgery.

During the medical consultation, the patient’s chest size
and the swelling of the areola and nipple
were clearly visible, showing obvious protrusion.

To ensure an accurate diagnosis,
an ultrasound examination was performed,
which revealed glandular tissue overgrowth in both breasts.
The patient was diagnosed with true gynecomastia,
classified as Simon Grade 2A.

Based on these findings,
our clinic proposed the following surgical plan:

1.Gland excision
Carefully remove the overgrown glandular tissue
to address the root cause of large chest size
and areola/nipple protrusion.

2.Liposuction
Remove fat from the lower and lateral chest
to create a flatter, smoother chest contour.

3.Adhesion and depression prevention
Fill the excised space evenly
to prevent skin–tissue adhesion
or nipple and areola indentation,
ensuring the tissue settles naturally.

Next, we will review the results
through 2-month postoperative photos
to see how the chest shape has changed.

Before gynecomastia surgery, both areola areas
were visibly protruding, and the firm, palpable
glandular tissue caused the chest line
to be noticeable even through clothing.

After removing the glandular tissue
and performing liposuction on the lower
and lateral chest, chest size was reduced
and the areola protrusion improved.

If the surgery had only reduced chest size,
the shape could have appeared unnatural.
However, by combining tissue repositioning
and liposuction designed to create
a masculine, three-dimensional chest,
the result was natural and firm in appearance.

In the before-and-after photos wearing a white T-shirt,
the concerns the patient had were completely resolved.

Before gynecomastia surgery, wearing thin clothes
made the chest volume visible, drawing attention
to the chest area and causing discomfort.

After surgery, even in a simple white cotton T-shirt,
the chest appears smooth and flat,
with a fit that looks athletic and masculine.

Thanks to these satisfying results,
the patient now enjoys a wider clothing selection,
and everyday stress has been greatly reduced.

For gynecomastia caused by multiple factors
such as puberty, weight gain, or hair loss medication,
precise diagnosis, surgical planning,
and consistent post-op management are essential.

If continued hair loss treatment is required,
the possibility of recurrence cannot be ruled out,
so regular follow-ups with medical staff
after surgery are strongly recommended.
