For men, chest concerns generally fall into two categories:
building chest muscles or reducing chest size.

The latter applies to men with gynecomastia,
a condition where the glandular tissue develops abnormally.

However, most patients are unaware of this fact
until they undergo an ultrasound examination
to confirm whether glandular overgrowth is present.

If the chest appears disproportionately enlarged,
or if consistent weight management
does not reduce the chest size,
there is a high chance that the cause is not fat,
but rather excess glandular tissue.

In this post, we will review the case of a patient
who visited Lesarts Plastic Surgery for this reason,
and explain in detail the diagnosis of gynecomastia
and the surgical process of male chest reduction surgery.

This patient, a man in his mid-30s,
had noticed chest protrusion on both sides
since his teenage years.

At first, the gynecomastia symptoms
did not cause much inconvenience.
However, as he entered adulthood,
the protrusion became more visible,
and the chest line and bulging areola
became noticeable through clothing.

Over time, he began to experience
both physical discomfort
and psychological stress
in his daily life,
caused by the persistent changes
in his chest shape.

During the examination, the patient’s chest
showed a clear protrusion centered on the nipples.
Skin elasticity was relatively good,
but fat accumulation in the lower chest and flanks
further emphasized the projection of glandular tissue.

Ultrasound confirmed glandular overgrowth
beneath both nipples, as well as the presence
of glandular tissue in the accessory breast (axillary area).
The diagnosis was true gynecomastia, Simon Grade 2B.

Based on this, the following
gynecomastia surgery plan was explained:

1.Gland Excision
Through a micro-incision around the areola,
the enlarged glandular tissue would be removed,
reducing chest size and correcting the enlarged areola.

2.Chest Liposuction
To prevent imbalance after gland removal,
liposuction was performed on the lower
and lateral chest to complete a flat, masculine contour.

3.Accessory Breast Tissue Removal
The glandular tissue in the axillary area was also removed
to refine the lateral chest line.

4.Adhesion & Depression Prevention
Careful tissue repositioning was applied
to prevent adhesions or indentations
in the space left after excision.

At 2 months post-surgery,
the patient’s gynecomastia symptoms improved
and the chest line transformation
was evaluated through before-and-after comparison photos.

Before gynecomastia chest reduction surgery,
the chest showed bulging volume centered on both nipples,
with a protruding chest line extending
down to the lower chest.

From the side view, the lower chest volume
appeared drooped, further emphasizing
an unbalanced chest contour.

At 2 months post-op, the glandular tissue
was excised, reducing overall chest size.
With liposuction of the lower and lateral chest,
surrounding bulk was refined,
reshaping the chest into a flatter, cleaner contour.

In particular, the nipple-area protrusion
was corrected, resulting in a more stable
and masculine chest shape
overall.

Even in everyday clothing,
the changes after gynecomastia chest reduction surgery
are clearly noticeable.

Before surgery, the protruding chest line
was fully visible under T-shirts.
Now, the chest appears much more natural
and maintains a stable, balanced fit.

Within just 2 months of recovery,
the chest protrusion was corrected,
and the T-shirt fit transformed.
This improvement was possible thanks to
a surgical plan that addressed the glandular tissue,
accessory breast tissue, and lower chest fat,
combined with consistent recovery management.

Looking ahead, if the patient maintains
