For men, having an unusually large chest
can cause not only physical discomfort
but also significant psychological stress
in daily life.

If the overall body is slim but the chest
appears disproportionately large,
it is often due to abnormal growth
of glandular tissue
, known as
gynecomastia, rather than fat accumulation.

In such cases, chest workouts or weight loss
do not solve the problem.

also had an average body frame,
but suffered from a prominent chest,
which became his greatest concern.

The patient noticed chest protrusion
since his teenage years.

Even after becoming an adult
and consistently managing his body,
the chest size did not decrease.
The area around the nipples appeared swollen,
and the chest projection was visible
through clothing, adding daily discomfort.

Despite efforts with exercise
and diet control, the chest problem
did not improve.
Instead, psychological stress increased,
leading the patient to finally visit

for an accurate diagnosis
and proper treatment.

Before surgery, the patient’s chest
showed a noticeable protrusion on both sides.
The nipples and areola appeared swollen,
with clear bulging observed.

On palpation, a firm glandular tissue lump
was felt beneath the nipples,
and ultrasound also confirmed
glandular overgrowth.

Based on these findings,
the patient was diagnosed with
true gynecomastia, Simon Grade 2A.

To treat gynecomastia,
the following surgical plan
was explained to the patient:

1.Gland Excision
Removal of enlarged glandular tissue
around the areola through direct excision,
reducing chest size.

2.Chest Liposuction
Since removing only glandular tissue
may cause contour imbalance
in the lower and lateral chest,
targeted liposuction was planned
to create a flatter, more masculine contour.

3.Adhesion & Depression Prevention
Tissue repositioning techniques were applied
to minimize risks of skin adhesion
or hollowing during recovery,
ensuring a smooth chest shape.

4.Hair-loss Medication & Recurrence Risk
As the patient was currently taking
medication for hair loss,
he was advised of the potential risk
of gynecomastia recurrence
and the importance of regular follow-ups.

At 2 months after gynecomastia surgery,
the degree of symptom relief
and improvement in the chest line
were evaluated through
before-and-after photos.

Before gynecomastia chest reduction surgery,
the chest showed prominent volume
centered around both nipples,
and the lower chest formed a rounded,
bulging contour with a heavy appearance.

From the side view, fat and glandular tissue
accumulated in the lower chest,
making the volume appear drooped downward.
The swelling of the nipples and areola
further emphasized the protrusion.

At 2 months post-op, after excision of
the glandular tissue and targeted liposuction
of the lower and lateral chest,
the chest contour became flat, neat,
and more masculine in shape.

In particular, the nipple protrusion subsided,
and the transition line from chest to upper body
settled into a smoother connection,
achieving a balanced, stable,
and naturally masculine contour.

Even in everyday clothing,
the changes before and after
gynecomastia chest reduction surgery
are clearly noticeable.

Before surgery, the protruding chest
was visible through T-shirts,
with bulging volume around the nipples
and a rounded lower chest line,
making the upper body look unnatural.

After surgery, with improvement
of gynecomastia symptoms and
liposuction of the lower and lateral chest,
the chest protrusion disappeared,
and the T-shirt fit settled into
a stable and natural contour.
