In such cases, a breast ultrasound can help determine if there is an overdevelopment of glandular tissue. If so, a combination of glandular tissue removal and, if necessary, liposuction may be required to improve the gynecomastia symptoms.

This patient is in his mid-30s and has been aware of his gynecomastia symptoms since his early 30s.

He experienced persistent discomfort in both daily life and social situations due to the protrusion of his chest, which led him to visit our clinic.

In an effort to alleviate the discomfort caused by gynecomastia, the patient consistently followed a diet plan and experienced some weight loss. He also noticed a slight reduction in the size of his chest.

However, despite these changes, he never fully achieved a flat chest appearance, and the discomfort persisted.

During the body assessment for gynecomastia surgery, the patient’s chest size was visually confirmed to be generally developed and protruding.

To accurately determine the cause of the gynecomastia, an ultrasound examination was conducted, revealing excessive development of the glandular tissue in both sides.

The patient was diagnosed with true gynecomastia, corresponding to Simon Grade 2A.

In this case, the recommended procedure was glandular tissue removal through gland excision, which would help address the chest volume issue that had caused discomfort.

To prevent the chest shape from becoming uneven or hollow due to the empty space left by the excised gland tissue, a tissue repositioning procedure was necessary to create a smoother and more refined chest shape.

However, the patient was taking vitamins, omega-3 supplements, and hair loss medication before the surgery.

Since vitamins, omega-3 supplements, and herbal remedies can increase the risk of bleeding during gynecomastia surgery, the patient was advised to stop taking these medications at least 5 days before the surgery after consulting with the medical team.

Furthermore, the hair loss medication that the patient was taking was explained to potentially affect hormone balance, which could lead to the possibility of gynecomastia recurrence. This was also discussed thoroughly with the patient.

As seen in the comparison images taken 2 months after the gynecomastia surgery, the volume of the chest, which was previously protruding due to the glandular tissue, has been improved, resulting in a more masculine chest shape.

Individuals with gynecomastia often prefer wearing dark-colored clothes, layering multiple garments, or opting for thicker fabrics to conceal the volume of their chest.

After gynecomastia surgery, the volume and protrusion of the chest are eliminated, allowing for a more masculine chest line with just a thin T-shirt.

While the patient did receive some help with fat reduction and skin tightening through diet and exercise, the main issue—the glandular tissue—could not be addressed until the surgery.

By eliminating this discomfort through gynecomastia surgery, the patient was able to regain a confident and masculine chest shape.
