Among the many physical changes women experience after childbirth,
changes in facial volume and overall facial appearance
are often just as noticeable as changes in body shape.

In particular, when the anterior cheeks lose volume
and the nasolabial folds become more pronounced,
the overall facial impression can change significantly.

Following pregnancy and childbirth, factors such as
fatigue, hormonal changes, and lifestyle adjustments
may contribute to facial volume loss, causing
the cheeks to appear more hollow and the facial contours
to lose their smooth, youthful definition.

The patient presented in this case
also noticed that her facial appearance had changed
after giving birth.

To restore the lost facial volume and regain
a healthier, softer, and more refreshed appearance,
she chose to undergo facial fat grafting.

When evaluating the patient’s concerns prior to facial fat grafting,
the anterior cheeks appeared relatively flat,
and the transition from beneath the cheekbones
to the nasolabial folds lacked smooth continuity.

Subtle shadowing was also visible in these areas,
making the midface appear flatter and contributing
to a slightly tired facial expression.

To address these concerns,
a facial fat grafting procedure was planned
to restore volume to the anterior cheeks,
nasolabial folds, and the region beneath the cheekbones.

Rather than creating excessive fullness,
the goal was to achieve natural three-dimensionality,
improve the continuity of the facial contour,
and restore a softer, more youthful, and harmonious appearance.

For this patient’s facial fat grafting procedure,
autologous fat harvested from the thighs was used.

The treatment focused on naturally restoring volume
to deficient areas while softening facial lines,
improving overall facial balance, and creating
a smoother, more harmonious contour.

Because the survival rate of transferred fat
varies from patient to patient,
the postoperative course was monitored closely
to evaluate long-term volume retention and
the overall contour changes before determining
whether additional treatment would be beneficial.

In this case, the anterior cheeks and
nasolabial fold region were initially corrected
through the primary fat grafting procedure.

After confirming satisfactory fat engraftment,
a secondary facial fat grafting procedure
was performed to refine subtle contour irregularities
and achieve optimal facial symmetry and balance.

The following before-and-after photographs
demonstrate the improvement in anterior cheek volume
and nasolabial contour, highlighting the natural
enhancement in facial harmony achieved through
staged facial fat grafting.

1 year after facial fat grafting

At 1 year after facial fat grafting,
the patient’s midface demonstrates
stable, natural volume centered in
the anterior cheeks, creating a softer
and more three-dimensional facial contour.

Before surgery,
the relatively flat anterior cheeks and
the shadow extending beneath the cheekbones
contributed to a tired and less vibrant appearance.

Following facial fat grafting,
the deficient volume of the anterior cheeks
has been naturally restored, improving the
central facial contour and creating a smoother transition
into the nasolabial fold region.

As a result, the face appears noticeably
brighter, healthier, and more refreshed.

The patient also expressed a high level
of satisfaction with the long-term outcome,
explaining that the uneven facial contours and
fatigued appearance that had concerned her
since childbirth had been significantly improved.

Rather than creating excessive facial fullness,
the procedure focused on subtle, targeted volume restoration,
enhancing only the areas that required correction.

This approach allowed her to maintain
her natural facial identity while achieving
a softer, younger, and healthier appearance.
