Create a beautiful S-line with hip dip fat transfer and thigh liposuction

Many women look in the mirror
and picture a “wannabe lower-body line.”
Not simply ultra-skinny legs,
but a waist and pelvic line
that draw soft curves,
and thighs connected from the pelvis
with a firm, long silhouette
without excess fat.
To achieve this kind of beautiful figure
often called an “S-line,”
what should we do?
Some people are born
with a good body frame and structure,
so they can achieve the line they want
through weight loss alone.
But for most people,
it’s true that weight loss and exercise
don’t bring fully satisfying results.
Especially when no amount of working out
can fix bumpy hip dips,
or inner thigh fat that just won’t separate,
these concerns go beyond
what dieting alone can solve.
In cases like this, what’s needed is
customized liposuction and fat transfer
planned with your body balance in mind.
The case I’m introducing today is
a patient who lost weight through strict self-management,
but because of body-type limitations,

After gaining weight, this patient worked consistently
on diet control and exercise
and lost a significant amount of weight on the scale.
However, she said that
the change in her body line
did not feel as satisfying
as the change in numbers.
Especially by visual observation,
the inner thighs were fully touching
from the upper area down to the knees
with no thigh gap at all,
which was causing her the most stress.

Also, the outer thighs had developed saddlebags
and hip dips at the same time.
Because of this, the line from the waist
to the pelvis and thighs looked uneven,
the pelvis appeared weak,
and the legs looked shorter and thicker.
On ultrasound,
there was not much fat around the knees
and hamstrings,
but other areas—especially the inner thighs—
had a lot of fat.
So we set a surgical plan
to address this while still creating
a natural, smooth line.

The patient was most concerned
about the inner thighs
and wanted “bone-thin thighs”
with a visible thigh gap.
She also wanted a fuller pelvic line
through hip dip fat transfer.
Checking the inner-thigh fat amount
Because the inner thighs were so tightly pressed together
that it felt almost suffocating,
we aimed to suction as much fat as possible.
However, if the pelvis is narrow
or the muscles are large,
even with maximum fat removal,
a thigh gap may not form.
We explained this in advance.
There was little fat around the knees,
but fortunately this area has fewer side effects,
so we decided to proceed
with the maximum safe amount of liposuction.
If too much fat is removed
from the middle inner-thigh area,
the inner thigh could sink in more than the knee,
making the line look curved
or the legs appear shorter.
So, to create a straight thigh-gap shape,
we decided to leave
a small amount of fat.
For the outer thighs,
we planned to remove as much fat as possible,
and at the same time transfer fat to the hip dip area
to create a smoother line.
If we performed the surgery
only to the extent of lightly filling the hip dips
without adding much fat,
it would have meant
a moderate and conservative amount of transfer.
However, since the patient wanted
a certain level of volume,
we decided to increase the transfer amount.
However, in hip dip fat transfer,
because the transfer volume is large,
if the placement is designed too low,
it can actually make the thighs look thicker.
So we transferred the fat
as high as possible
to prevent the thighs
from looking thick and short.

Before surgery, the patient’s thighs were overall thick,
with fat especially concentrated on the inner thighs.
The pelvic line was also uneven,
and there were hip dips,
which made the pelvis look flat
and created the visual downside
of legs appearing thicker and shorter.
Based on the patient’s request,
we worked to create as much thigh gap as possible
on the inner thighs.
Since she wanted a “bone-thin thigh” look,
we proceeded with the maximum safe suction volume as the baseline,
and combined hip dip fat transfer
to create a fuller, more contoured line
from the pelvis to the outer thighs.
However, as expected before surgery,
the connection line did not align perfectly
all the way into the saddlebag area.
Still, the patient was satisfied enough
to feel there was no need
to remove the saddlebags further
or add more fat transfer,
so no additional transfer was performed.

For the inner thighs,
it was expected that a perfect thigh gap
would be difficult to create due to her body type.
However, fortunately a gap formed
from the knees up to the upper thighs,
and the legs changed into a look
that no longer felt tight or stuffy.
By adjusting the suction volume
at each key point,
the thigh-gap line was also shaped
into a smooth, straight form,
making the legs look much longer and slimmer.
For the outer thighs,
a voluminous line from the pelvis to the thighs
connected softly and naturally.
The pelvis looks wider, the legs look longer,
and the lower-body line became elegant.

The thighs that had protruded forward a lot
became much slimmer
with a gentler curve,
and for the back of the thighs,
as expected before surgery,
the line was refined in harmony
rather than showing a dramatic change.
Because this patient wanted
a “bone-thin thigh” style,
we removed as much fat as possible
within a safe range
without causing side effects.
By adding fat transfer to the hip dips,
she was able to achieve
a softer, fuller pelvic line
and a more feminine lower-body contour.
However, if her weight increases later,
the balance between the upper and lower body
could be affected,
so I advised her
to manage her weight carefully.
