[Arm liposuction] Even after weight loss, the upper body stays the same, a solution to end the struggle that dieting alone cannot solve
Hello, this is Dr. Yongseung Seo,
from Lesarts Plastic Surgery, specializing in
liposuction and fat transfer surgery.
When consulting many working women,
even with strict dieting schedules
and consistent exercise routines,
they share the same frustration.
“My weight has decreased,
but my arm circumference
barely changes at all.”
“My arms still look thick,
so my whole upper body
appears larger and heavier,
and getting dressed feels stressful.”
These concerns are expressed
far more frequently
than most people expect
during daily consultations.
Many try intense arm workouts
to burn stubborn arm fat,
but the muscles grow instead,
making the arms look thicker.
At the same time,
fat from unintended areas
may decrease too much,
causing additional imbalance and stress.
Even after dedicated dieting,
body weight goes down,
yet the upper body silhouette
still looks wide and bulky.
The stubborn upper arm fat
barely changes at all,
so patients eventually seek
arm liposuction consultation.
Today’s patient visited
Lesarts Plastic Surgery seeking
a solution for stubborn arm fat
that never improved with dieting.
She is 160 cm tall,
weighing 81.3 kg,
with a BMI of 31.8,
classified as lower body obesity.
In the past, she achieved
significant weight loss
through strict dieting
and intense exercise efforts.
However, despite those changes,
her upper arm circumference
barely decreased at all,
which caused deep frustration.
After regaining weight over time,
she reached her highest weight,
and felt her upper body
looked even larger than before.
During consultation, she said
she simply wanted
to end this long-term stress
and physical discomfort.
[Patient information]
• Age: late 20s
• Height: 160 cm
• Weight: 81.3 kg (highest)
• BMI: 31.8 kg/m²
• Body fat: 42.4%
Even with similar body specs,
liposuction results may vary,
depending on individual fat distribution,
skin elasticity, and healing response.

Arm liposuction consultation and diagnosis – Lesarts Plastic Surgery – Dr. Yongseung Seo
Consultation and assessment results
“Even after losing weight,
my arm circumference
barely decreases at all.”
When her arms were relaxed
and naturally lowered,
a noticeable posterior bulge
was clearly visible.
The fat silhouette
was prominent enough
to be seen
even from the front view.
A significant amount of
stubborn upper arm fat
had accumulated throughout
the entire arm region.
Because of this excess fat,
visible skin sagging
became more severe
when lifting the arms.
These findings indicated
that dieting alone
would not sufficiently improve
the arm contour.
Sagging appearance when lifting the arms
In addition, excess fat
was also present around
the axilla and scapular areas,
making the upper body appear bulkier.
According to the ultrasound scan,
the thickness of arm fat
was measured as follows.
Posterior arm: 25 / 26 mm
Anterior arm: 18 / 18 mm
Scapular area: 21 / 25 mm
These values indicated
a significant fat volume,
so the expected liposuction effect
was considered substantial.
However, because the fat layer
was relatively thick,
the risk of postoperative skin laxity
was also quite high.
We carefully explained
the possibility of sagging
and the importance of
post-surgical skin tightening care.
The shoulder muscles were
also highly developed,
making a sharp, straight
right-angle arm line difficult.
Additionally, long-standing stretch marks
may improve slightly,
but complete removal
is realistically impossible.
Upper body design for high fat and muscle mass
In this patient,
not only fat volume
but also muscle mass
was significantly developed.
If fat were removed aggressively
without considering muscle shape,
the arm contour could collapse
and results might appear unnatural.
Therefore, rather than
simple maximum fat removal,
precise contour refinement
based on muscle anatomy was essential.
A tailored upper body design
focusing on balance and proportion
was the key point
of this surgical plan.
Upper body liposuction physical examination and surgical plan – Lesarts Plastic Surgery – Dr. Yongseung Seo
1. Upper arms
To achieve visible
arm circumference reduction,
sufficient fat removal
is clearly necessary.
However, perceived thickness
is determined mainly
by the thickest arm area,
not the overall volume alone.
Around the shoulder,
where muscles are naturally bulky,
excessive fat removal
may expose harsh borders.
Therefore, even with
maximum arm liposuction,
fat must be finely adjusted
around the muscle boundary.
This careful contour control
helps create smoother lines
and a slimmer,
more refined arm shape.
2. Scapula and axilla
When upper body fat
is heavily accumulated,
the scapula and axilla
should be treated together.
By combining axilla + scapula liposuction,
the overall upper body size
can be reduced
for better arm proportions.
Reducing bulging underarm fat
also prevents compression
when the arms rest
against the torso.
This allows cleaner lines
when the arms are lowered,
making the arms appear
longer and slimmer visually.
3. Limitations
However, due to
significantly developed
shoulder muscles,
a sharp right-angle arm is unrealistic.
Because of the thick
subcutaneous fat layer,
temporary sagging may occur
after surgery.
This can create
a slightly unnatural contour
during early recovery,
so skin tightening care is essential.

Before arm liposuction,
the posterior arm area
protruded noticeably,
making the upper body appear shorter and heavier.
After surgery,
the posterior bulge
was significantly reduced,
creating a cleaner arm contour.
As the excess fat was removed,
the arm line appeared
much straighter and longer,
enhancing the overall silhouette.
The patient also shared
that when lowering her arms,
the outer line looked smooth
and naturally straight.
She mentioned it felt amazing
that even tight long-sleeve shirts
now fit in a clean,
straight slim-fit shape.

Contrary to initial concerns,
even when lifting the arms,
there was no significant
downward skin sagging.
Instead, a slimmer
and more refined
arm contour
was clearly maintained.
How to lose arm fat – arm, axilla, and scapula liposuction – Lesarts Plastic Surgery – Dr. Yongseung Seo
As the axilla and scapula fat
were reduced together,
the overall upper body circumference
visibly decreased as well.
With improved proportions
between the arms and torso,
the previously bulky
upper body silhouette softened noticeably.
In many cases like this,
patients visit us saying,
“Only my stubborn arm fat
never seems to go away.”
While liposuction can certainly help,
focusing only on removing
as much fat as possible
can be very risky.
Excessive, aggressive fat removal
may lead to complications
that are often
difficult to reverse.
Even without side effects,
if balance and proportions collapse,
the final contour may look worse
than before surgery.
Therefore, it is crucial
to predict possible changes,
understand limitations in advance,
and prepare proper solutions.
If your upper body line
does not improve despite
your busy daily efforts,
you do not have to struggle alone.
Please contact Lesarts,
and we will help you
find a safe and effective solution
to finally end that stress.
