to assess the
upper arm contour.
This posture helps reveal
fat distribution
around the upper arms,
axillary area,
and surrounding regions.
Patient Information
Age: Late 30s
Height: 158 cm
Weight: 69.2 kg
BMI: 27.7 kg/m²
Body Fat Percentage: 44.9%
Even with similar
body measurements,
results of liposuction surgery
may vary depending
on individual conditions.

Before surgery,
the patient was informed
that priorities during
a wedding ceremony
may differ from daily life.
When evaluating
the arm contour,
we usually assess
the arms in
a relaxed standing posture.
However, during
a wedding ceremony,
the bride often holds
a bouquet
with arms gently bent.
Because of this,
the outer arm line
that matters in daily life
may appear less
prominent in wedding photos.
Among surrounding areas,
the scapular region
is often considered
the most important
for upper body balance.
Depending on
the dress design,
the accessory breast area
may also become
visually significant.
In contrast,
the axillary area
usually plays
a relatively minor role
in the overall appearance.
After discussing
these differences,
we determined
the surgical focus
based on the patient’s priorities.
For the arms,
the goal was
a clean and delicate
straight arm line.
For surrounding areas,
since there was
no concern about
the scapular region,
we decided to refine
only the axilla
and accessory breast areas.

The patient presented
with relatively high
muscle mass
in the upper arm
and shoulder area.
If fat were removed
aggressively without
considering the contour,
the muscle definition
could become overemphasized.
This could result
in a stronger
“athletic arm” appearance
rather than the desired
delicate straight arm line.
Therefore, some fat
along the muscle borders
was intentionally preserved
to create a smoother
and softer silhouette.
Overall fat accumulation
was still significant,
and repeated weight fluctuations
had reduced
skin elasticity.
While noticeable
before-and-after change
was expected,
the skin might appear
slightly loose
after fat removal.
This possibility
of mild skin laxity
or wrinkling
was explained
before surgery.
Additionally, the patient
had relatively large
deltoid muscles
measuring about 22/19 mm.
Because of this,
creating a perfectly
right-angle arm line
would be difficult.
There was also
mild left–right asymmetry,
which could cause
differences in thickness
between both arms.

2. When the Arms Are Raised
The previously sagging
posterior arm fat
that hung downward
has largely
been reduced.
Although skin elasticity
was somewhat reduced,
the fat could not
be removed
completely.
However, the before-and-after difference
is clearly noticeable,
with the arms appearing
significantly thinner
than before surgery.
The axillary contour
also appears deeper,
which enhances
a more slender
overall impression.
As this was

we focused on creating
