After pregnancy and childbirth, even when overall condition
returns with time, sagging abdominal skin often does not
recover as easily as expected.

This skin elasticity issue tends to persist or worsen
over time, sometimes becoming a constant discomfort
in daily life.

No matter how much patients try to manage it,
these changes in body shape, skin quality,
and abdominal structure
rarely return
to the pre-pregnancy state, leading many
to seriously consider surgical solutions.

An abdominoplasty (tummy tuck) is one of the main
methods to address these elasticity problems
and help restore the body closer
to its original shape.

Today’s case involves a patient in her 40s
who has experienced two natural childbirths.
She maintained her weight steadily over the years,
but sagging skin in the lower abdomen
caused folds and visible creases.

The central abdomen appeared to bulge forward
in a way that felt different from simple fat gain.
This protrusion created significant stress
when wearing clothing, as it disrupted fit
and made the belly look more prominent.

After meals, the upper and lower abdomen
felt even more distended, causing pants
to feel uncomfortably tight and preventing
her from achieving her desired silhouette.

She recognized these issues of sagging skin
and abdominal structure as separate from fat gain.
Seeking an accurate diagnosis, including whether
rectus diastasis (muscle separation) was present,
she visited Lesarts Plastic Surgery for consultation.

During the pre-surgery consultation for abdominoplasty,
the patient’s abdominal sagging was carefully examined.
It was confirmed that after two pregnancies and deliveries,
skin elasticity had significantly weakened, and stretch marks
were widely spread across the upper abdomen.

The abdominal muscles had separated,
causing the entire abdomen to bulge forward.
In addition, sagging skin pulled the navel downward,
lowering its position unnaturally and disrupting
overall abdominal balance and proportions.

Since both skin laxity and abdominal protrusion
were present together, improvement was needed
for both the inner abdominal wall and the outer contour.
The surgical plan was made to restore elasticity
and achieve a balanced abdominal line.

1.M-Type Abdominoplasty
Sagging skin and stretch marks in the upper abdomen
were tightened and lifted with M-Type abdominoplasty.
The drooping navel was reconstructed and repositioned
to restore the ideal abdominal center line.

Because the gap between the rectus muscles was wide,
rectus plication was essential. Using the Triple Lock
technique, the abdominal bulge was corrected,
creating a slimmer waistline and improved contour.

2.Abdominal Liposuction
Although the patient did not have excessive fat,
liposuction was added to refine the waistline
and flanks for smoother definition.
Together, these procedures provided a harmonious
and proportional transformation of the abdomen.

Below, we review the results from 1 to 3 months
after surgery to observe how the waistline improved
following the tummy tuck and liposuction.

Before surgery, the lower abdominal skin sagged downward,
creating folds and making the entire abdomen look heavy.

By 1 month after the tummy tuck, abdominal elasticity
had improved, the repositioned navel emphasized the waistline,
and swelling reduction revealed a firmer contour.

As the recovery progressed, the abdomen gradually flattened,
showing a smoother overall shape. The previously bulging
abdominal wall was corrected, creating a natural transition
toward the waistline and forming a firm, balanced silhouette.

From the oblique view before abdominoplasty,
the protruding abdominal contour and sagging skin
gave a heavy and unbalanced impression.

After surgery, with rectus muscle correction,
the abdominal center was pulled inward,
the folds in the lower abdomen disappeared,
and the overall heaviness was visibly improved.

Even when bending forward at the waist,
the sagging skin that once drooped downward
due to gravity is now lifted firmly.

After abdominoplasty, the abdomen maintains
a stable and smooth contour without folds,
showing improved tension and shape
even in a bent posture.

From the side view before tummy tuck surgery,
the bulging abdominal wall projected forward,
creating an uneven and unstable contour.

Sagging skin starting from the upper abdomen
continued downward, adding heaviness
and disrupting the natural abdominal line.

After abdominoplasty, the separated rectus muscles
were corrected, eliminating the abdominal bulge
and restoring a firm, balanced contour in front.
The sagging skin also gained elasticity,
resulting in a lighter, more refined abdomen.

As seen in this case, even with consistent diet
and exercise, structural issues like stretched skin
and muscle separation from pregnancy and childbirth
cannot always be reversed without surgical support.
Surgery provided clear and lasting improvement.

The patient’s recovery has progressed smoothly,
without complications, and she was advised
on continuous care, including scar management
with silicone bands, hydration, and self-care routines
.

Because a tummy tuck addresses not only aesthetics
but also abdominal function and body balance,
attentive care during recovery ensures
stable and satisfying results that last long term.
